Abstract
Evaluasi kemoterapi wajib dilakukan secara siklusal untuk menilai hasil terapi. Saat ini CT scan toraks dengan kontras menggunakan Response Evaluation Criteria in Solid Tumours (RECIST 1.1) menjadi standar baku untuk mengetahui kanker paru progresif atau non progresif. Interval waktu antara pemindaian CT scan toraks siklusal yang terlalu lama dan keterbatasan alat termasuk efek samping masih menjadi hambatan dalam evaluasi serta mendeteksi lebih awal kegagalan terapi. Tujuan penelitian ini untuk mengetahui hubungan antara pola senyawa Volatile Organic Compound (VOC) dan Interleukin-23 (IL-23) terhadap RECIST 1.1 dalam evaluasi respon kemoterapi pada pasien kanker paru di Rumah Sakit Saiful Anwar. Penelitian observasional analitik dengan desain kohort prospektif pada 47 pasien kanker paru yang tidak pernah mendapatkan pengobatan kanker sebelumnya (naive), diikuti mulai awal pengobatan kemoterapi platinum base sampai akhir siklus ke-3, pengukuran VOC menggunakan alat μβreath. Pada 7 jenis VOC yang diteliti, 4 diantaranya mengalami penurunan kadar sesudah mendapatkan 3 siklus kemoterapi, yaitu: Formaldehid (CH2O), Toluena (C7H8), Aseton (C3H6O),dan Heksana (C6H14). Selain itu, terdapat hubungan yang bermakna antara C3H6O dengan RECIST 1.1 yang berkorelasi negatif (p=0.023) dan antara Metana (CH4) dengan RECIST 1.1 yang berkorelasi positif (p=0.011). Di sisi lain, pemeriksaan IL-23 sebelum dan sesudah kemoterapi 3 siklus tidak memiliki perbedaan yang bermakna, namun kadar IL-23 post-kemoterapi memiliki hubungan bermakna dengan RECIST 1.1 dan berkorelasi positif (p= 0.000). Kesimpulan dari penelitian ini peningkatan CH4 dan IL-23, serta penurunan C3H6O menunjukkan peningkatan progresivitas pada kanker paru yang dilakukan kemoterapi.
English Abstract
Chemotherapy evaluation must be performed siklusally to assess the results of therapy. Currently, a chest CT scan with contrast using Response Evaluation Criteria in Solid Tumors (RECIST 1.1) is the gold standard for identifying progressivity lung cancer. The long time interval between siklusal chest CT scans, tool limitations and the side effects are still a barrier to evaluation and early detection of therapeutic failure. This study aims to determine the correlation between the pattern of Volatile Organic Compound (VOC) and Interleukin-23 (IL-23) to RECIST 1.1 in evaluating chemotherapy responses in lung cancer patients at Saiful Anwar Hospital. Analytical observational study with a prospective cohort design in 47 lung cancer patients who had never received previous cancer treatment (naïve) were followed from the beginning of platinum-based chemotherapy to the 3rd sikluses. VOC measurement using the μβreath tool. 7 types of VOCs were studied and 4 of them had decreased levels after receiving 3 sikluses of chemotherapy, namely: Formaldehyde (CH2O), Toluene (C7H8), Acetone (C3H6O), and Hexane (C6H14). In addition, there is a significant correlation between C3H6O and RECIST 1.1 which has a negative correlation (p=0.023), and between Methane (CH4) and RECIST 1.1 which has a positive correlation (p=0.011). Furthermore, IL-23 examination before and after 3 sikluses of chemotherapy had no significant difference. Nevertheless, post-chemotherapy IL-23 levels had a significant correlation with RECIST 1.1 which positively correlated (p=0.000).he increases in CH4 and IL-23, and decreases in C3H6O indicate the increased progression in lung cancer undergoing chemotherapy.
