Abstract
Pasien Re-bleeding post craniectomy atas indikasi ICH mengalami gangguan sistem pernafasan sehingga membutuhkan suplay oksigen yang adekuat dan biasanya diperlukan tindakan pemasangan ventilator. Pada pasien-pasien dengan ventilator dan trakeostomi sering mengalami masalah ketidakefektifan bersihan jalan nafas. Pada penelitian ini peneliti menggunakan jenis penelitian deskriptif dengan metode studi kasus pada Ny A dengan diagnosis ICH yang mengalami re-bleeding Post-op Craniectomy dengan masalah keperawatan bersihan jalan nafas tidak efektif yang dirawat diruang ICU (Intensive Care Unit) RS Tk. II Dr. Soepraoen Malang. Intervensi yang diberikan berupa Fisioterapi dada clapping dan vibrating selama 3-5 menit sebagai salah satu tatalaksana dari masalah bersihan jalan nafas tidak efektif. Setelah diberikan 5x intervensi bunyi napas tambahan masih ada namun sudah lebih baik daripada sebelum diberikan intervensi, produksi sputum berkurang, kualitas sputum membaik, frekuensi pernapasan dalam batas normal, saturasi oksigen meningkat, pasien mampu mengeluarkan sekret tanpa perlu dilakukan suction. Pemberian fisioterapi dada clapping dan vibrating selama 3-5 menit pada segmen parudapat meningkatkan bersihan jalan nafas pasien. Intervensi tersebut cukup efektif dalam membantu pasien mengeluarkan sekret yang menumpuk pada paru-paru.
English Abstract
Re-bleeding post craniectomy patients with ICH indications experience respiratory system disorders so they require adequate oxygen supply and usually require a ventilator. Patients with ventilators and tracheostomies often experience problems with ineffective airway clearance. In this study, the researcher used a descriptive research type with a case study method in Mrs. A with a diagnosis of ICH who underwent re-bleeding Post-op Craniectomy with ineffective airway cleaning nursing problems and was treated in the ICU (Intensive Care Unit) room of Tk Hospital. II Dr. Soepraoen Malang. The intervention given was in the form of clapping and vibrating chest physiotherapy for 3-5 minutes as a treatment for ineffective airway clearance problems. After being given 5x interventions additional breath sounds are still present but better than before intervention, sputum production is reduced, sputum quality is improving, respiratory rate is within normal limits, oxygen saturation is increasing, and the patient can remove secretions without the need for suction. Giving chest physiotherapy clapping and vibrating for 3-5 minutes on the lung segments can improve the patient's airway clearance. This intervention is quite effective in helping patients remove secretions that have accumulated in the lungs.
