Hubungan Antara Volatile Organic Compounds (VOC) dengan Leukotrien B4 dan Neutrofil Serum pada Pasien PPOK

Abstract

Latar belakang : PPOK diperkirakan menjadi penyebab kematian terbanyak ketiga di dunia pada tahun 2030. PPOK dapat menyebabkan inflamasi sistemik kronik yang menyebabkan komplikasi ekstrapulmoner. Spirometri masih menjadi standard diagnostik PPOK, namun membutuhkan manuver khusus dan pemeriksa yang kompeten. Leukotrien B4 dan neutrofil juga berperan dalam patogenesis PPOK. Pada beberapa dekade terakhir dikembangkan metode non invasif berupa pemeriksaan Volatile Organic Compounds (VOC) untuk deteksi penyakit paru. Penelitian ini bertujuan untuk menganalisa perbedaan kadar VOC pada udara pernapasan pasien PPOK dengan kelompok kontrol, serta menganalisa hubungan kadar VOC dengan Leukotrien B4 dan neutrofil serum pasien PPOK. Metode : Penelitian case control dilakukan dengan total 40 subjek terdiagnosis PPOK stabil dan 40 subjek kontrol. Sampel udara pernapasan dikumpulkan untuk pemeriksaan kadar VOC dan darah vena untuk pemeriksaan Leukotrien B4 dan neutrofil. Sampel udara kemudian dianalisa menggunakan alat breath analyzer untuk menilai kadar CO2, C2H5OH, CH2O, C7H8, C3H6O, NH4, C6H14, NO2, CO, NH3, CH4, C6H6, dan C3H8. Hasil : Terdapat perbedaan signifikan (p<0,05) dari CO2, C2H5OH, C3H6O, NO2, CO, NH3, C6H6, dan C3H8 antara kelompok PPOK dan kontrol. Analisa lanjutan menunjukkan korelasi signifikan antara C2H5OH, CO, dan C6H6 dengan Leukotrien B4. Akan tetapi, tidak ada VOC yang berkorelasi dengan neutrofil serum secara statistik. Kesimpulan : Kadar beberapa VOC ditemukan berbeda pada pasien PPOK dibandingkan kelompok kontrol. VOC berkorelasi dengan kadar Leukotrien B4 serum pada PPOK.

English Abstract

Background: COPD has been estimated become the third leading cause of death globally in 2030. COPD can promote chronic systemic inflammation causing extrapulmonary complications. Spirometry is still diagnostic standard for COPD, but it needs certain maneuvers and trained operator. Leukotriene B4 and neutrophil are also involved in COPD pathogenesis. In last few decades, the development of non invasive method for detecting lung disease has emerged, such as Volatile Organic Compounds (VOC) examination. This study aims to analyze the difference of VOC in exhaled breath of stable COPD and healthy subjects, furthermore the correlation of VOC between serum Leukotriene B4 and neutrophil. Methods: A case-control study was conducted recruiting 40 stable COPD patients and 40 healthy subjects. Exhaled breath sample was collected for VOC detection and venous blood sample for examining Leukotriene B4 and neutrophil. Breath sample was analyzed using an arrayed sensor breath analyzer to check the concentration of CO2, C2H5OH, CH2O, C7H8, C3H6O, NH4, C6H14, NO2, CO, NH3, CH4, C6H6, and C3H8. Results: There were significant difference (p<0.05) of CO2, C2H5OH, C3H6O, NO2, CO, NH3, C6H6, dan C3H8 parameters between COPD patients and healthy subjects. Further analysis showed significant correlation among C2H5OH, CO, C6H6 and Leukotriene B4. Nevertheless, there was no VOC correlated with serum neutrophil in this study. Conclusion: Level of some VOC in COPD patients differ from healthy subjects. Furthermore, VOC is also correlated with serum Leukotriene B4 in COPD.
