Abstract

Pendahuluan: Nefropati diabetik merupakan salah satu penyakit komplikasi dengan insidensi dan mortalitas yang tinggi. Nefropati diabetik ditandai adanya albuminuria dan peningkatan receptor advanced glycation end products (RAGE). Pemberian senyawa herbal asam rosmarinat (AR) dan antihipertensi golongan angiotensin receptor blocker (valsartan dan irbsartan) diketahui memiliki efek nefroproteksi. Namun, efek dari kombinasi tersebut masih belum pernah diteliti. Tujuan: Mengetahui perbedaan efek valsartan dan irbesartan terhadap ekspresi Receptor for Advanced Glycation End Product (RAGE) jaringan ginjal dan kadar albuminuria pada tikus model diabetik yang mendapatkan asam rosmarinat. Metode: True experimental laboratory dan metode randomized post-test only controlled group design. Tikus dibagi menjadi 4 kelompok yang terdiri dari kelompok kontrol positif, kontrol negatif, perlakuan 1 (AR+valsrtan) dan perlakuan 2 (AR+Irbesartan). Kadar albuminuria diukur menggunakan enzyme linked immunoabsorben assay sedangkan ekspresi RAGE diukur menggunakan imunohistokimia. Hasil dianalisa menggunakan One-Way ANOVA dilanjutkan Pos Hoc LSD dengan tingkat signifikasi p<0,05. Hasil: Pemberian kombinasi AR+ Valsartan atau AR+Irbesartan menurunkan secara signifikan ekspresi RAGE (p<0,001; p<0,001) dan pemberian kombinasi AR+ Valsartan menurunkan secara signifikan kadar albuminuria (p=0,006) namun AR+Irbesartan tidak signifikan menurunkan kadar albuminuria (p=0,074) dibandingkan kontrol positif. Tidak terdapat perbedaan signifikan kombinasi AR+Valsartan dan AR+Irbesartan dalam menurunkan ekspresi RAGE (p=0,413) dan kadar albuminuria (p=0,148). Kesimpulan: Pemberian AR+ Valsartan atau AR+Irbesartan menurunkan secara signifikan ekspresi RAGE. Pemberian kombinasi AR+Valsartan signifikan menurunkan kadar albuminuria namun kombinasi AR+Irbesartan tidak signifikan menurunkan kadar albuminuria.

English Abstract

Introduction: Diabetic nephropathy is a complicated disease with the highest incidence and mortality. Diabetic nephropathy begins characterized by albuminuria and an increase in receptor advanced glycation end products (RAGE). Administration of the herbal compound rosmarinic acid (AR) and antihypertensive angiotensin receptor blockers (valsartan and irbsartan) are known to have a nephroprotective effect. However, the effects of this combination have never been studied. Objective: To determine the differences in the effects of valsartan and irbesartan on Receptor for Advanced Glycation End Product (RAGE) expression in the kidneys and albuminuria levels of diabetic rats receiving rosmarinic acid. Method: True experimental laboratory and randomized post-test only controlled group design method. Mice were divided into 4 groups consisting of positive control, negative control, treatment 1 (AR+valsrtan) and treatment 2 (AR+Irbesartan). Albuminuria levels were measured using enzyme linked immunoabsorbent assay while RAGE expression was measured using immunohistochemistry. The results were analyzed using One-Way ANOVA followed by Post Hoc LSD with a significance level of p<0.05. Results: The combination of AR+Valsartan or AR+Irbesartan significantly reduced RAGE expression (p<0.001; p<0.001) and the combination AR+Valsartan significantly reduced albuminuria levels (p=0.006) but AR+Irbesartan not significantly reduced albuminuria levels (p=0.074) compared to the positive control. There was no significant difference between the combination of AR+Valsartan and AR+Irbesartan in reducing RAGE expression (p=0.413) and albuminuria levels (p=0.148). Conclusion: Administration of a combination of AR+Valsartan or AR+Irbesartan significantly reduces RAGE expression. the combination AR+Valsartan significantly reduced albuminuria levels but AR+Irbesartan not significantly reduced albuminuria levels
