Abstract

Pendahuluan dan Tujuan: Penyakit Paru Obstruktif Kronik (PPOK) merupakan gangguan pernapasan progresif yang ditandai oleh peradangan paru kronis, yang memerlukan penilaian tingkat keparahan penyakit yang akurat untuk pengelolaan yang efektif. Volatile Organic Compounds (VOCs) menunjukkan potensi sebagai biomarker non-invasif, mencerminkan patofisiologi PPOK. Penelitian bertujuan untuk menyelidiki korelasi antara VOCs dan kadar Leukotriene B4 (LTB4), serta jumlah eosinofil serum pada pasien PPOK. Metode: Menggunakan metode observasional-analitik dengan pendekatan kasus-kontrol, sebanyak 40 pasien PPOK dan 40 subjek kontrol diikutsertakan dari departemen rawat jalan paru Rumah Sakit Dr. Saiful Anwar, Malang. Kadar VOCs diukur menggunakan analisis napas, sementara kadar LTB4 ditentukan melalui uji enzim terikat imunoserbent. Uji korelasi Spearman digunakan untuk melihat hubungan antara VOCs, LTB4, eosinofil serum, dan komorbiditas, dengan uji Mann-Whitney membandingkan hasil dengan kelompok kontrol. Data dianggap signifikan apabila p < 0,05. Hasil: Terdapat 40 pasien PPOK dan 40 subjek kontrol dalam penelitian ini. Ditemukan perbedaan signifikan antara kadar VOCs pada kelompok PPOK dan kontrol (p < 0,05). Kadar LTB4 secara signifikan lebih tinggi pada pasien PPOK dibandingkan subjek kontrol (p < 0,001), sedangkan tidak terdapat perbedaan kadar eosinofil. Terdapat korelasi antara LTB4 dan kadar VOC senyawaviii C2H5OH pada pasien PPOK (p = 0,009; r = 0,410). Namun, tidak terdapat korelasi antara eosinofil serum dan VOCs (p = 0,939). Kadar VOCs juga berbeda secara signifikan antara PPOK dan pasien PPOK dengan komorbiditas kanker paru (p < 0,05). Kesimpulan: Terdapat korelasi antara kadar VOC dan LTB4 pada pasien PPOK.

English Abstract

Introduction and Purpose: Chronic Obstructive Pulmonary Disease (COPD) is a progressive respiratory disorder characterized by chronic lung inflammation, necessitating accurate disease severity assessment for effective management. Volatile Organic Compounds (VOCs) have shown promise as non-invasive biomarkers, reflecting COPD pathophysiology. This study aimed to investigate the correlation between VOCs and Leukotriene B4 (LTB4) levels, as well as serum eosinophil counts in COPD patients. Method : Using an observational-analytic method with a case-control approach, 40 COPD patients and 40 controls were enrolled from the respiratory outpatient department of Dr. Saiful Anwar Hospital, Malang. VOC levels were measured using a breath analyzer, while LTB4 levels were determined through enzymelinked immunosorbent assays. Spearman correlation tests examined associations between VOCs, LTB4, serum eosinophil, and comorbidity, with Mann-Whitney tests comparing results against the control group. Data significance was set at p < 0.05. Result : There were 40 COPD patients and 40 control in this research. There were significant difference between VOCs in COPD and control group (p<0,05). LTB4 level significantly increased in COPD than control group (p<0,001) and no difference of eosinophil level. There was a correlation between LTB4 and VOCx level of C2H5OH in COPD patients (p=0,009; r=0,410). There was no correlation between serum eosinophil and VOCs (p=0,939). The level of VOCs significantly different between COPD and COPD with lung cancer comorbid patients (p<0,05). Conclusion: There were a correlation between VOC and LTB4 in COPD patients
