Abstract
Skizofrenia adalah gangguan jiwa yang ditandai dengan gangguan pada pola pikir, proses persepsi, keterikatan, dan perilaku sosial. Gejala skizofrenia dibagi menjadi dua kategori, positif dan negatif. Salah satu gejala positif klien skizofrenia adalah adanya risiko gangguan perilaku seperti risiko perilaku kekerasan. Penerapan asuhan keperawatan dilakukan di rumah klien yang berada di desa Kuwolu lingkup Puskesmas Bululawang. Asuhan keperawatan yang diterapkan merupakan salah satu penanganan pada seseorang yang mengalami risiko perilaku kekerasan meliputi pengkajian, analisa data, intervensi, implementasi dan evaluasi keperawatan serta melakukan 1-4 Strategi Pelaksanaan (SP) Risiko Perilaku Kekerasan. Hasil pengkajian dari keterangan keluarga didapatkan awal sakit klien dikarenakan keluar dengan temannya dan ketika pulang klien menjadi sering merusak barang di sekitar rumah. Keluarga mengatakan klien tidak pernah melukai orang lain, hanya saja merusak barang yang dapat melukai dirinya sendiri. Analisa keperawatan didapatkan diagnosa keperawatan yang utama risiko perilaku kekerasan, gangguan persepsi sensori (halusinasi penglihatan), penyangkalan tidak efektif, manajemen kesehatan tidak efektif, dan koping tidak efektif. Rencana tindakan keperawatan yang akan dilakukan yaitu Strategi Pelaksanaan (SP) Risiko Perilaku Kekerasan 1-4 sebagai masalah utama yang diangkat. Implementasi keperawatan dilakukan selama 4 kali pertemuan dengan klien dan keluarga. Setelah 4 kali pertemuan, SP pada klien hanya sampai SP 2 dan keluarga sampai SP 4. Hasil evaluasi didapatkan terdapat perubahan yang positif selama pertemuan dengan klien. Asuhan keperawatan dengan risiko perilaku kekerasan belum dapat dituntaskan dikarenakan jangka waktu yang pendek dan kurang kooperatifnya klien. Karya ilmiah akhir ini diharapkan dapat menjadi acuan untuk menangani pasien dengan diagnosa keperawatan Risiko Perilaku Kekerasan.
English Abstract
Schizophrenia is a mental disorder characterized by disturbances in thought patterns, perceptual processes, attachments, and social behavior. Schizophrenia symptoms are divided into two categories, positive and negative. One of the positive symptoms of schizophrenic clients is the risk of behavioral disorders such as the risk of violent behavior. The application of nursing care is carried out at the client's house in the Kuwolu village within the Bululawang Health Center. Nursing care that is applied is one of the treatments for someone who is at risk of violent behavior including assessment, data analysis, intervention, implementation and evaluation of nursing as well as carrying out 1-4 Implementation Strategies for the Risk of Violent Behavior. The results of the study, based on family information, showed that the client was sick because he was out with his friends and when he came home, the client often broke things around the house. The family said that the client had never injured anyone else, only damaged things that could injure himself. Nursing analysis found that the main nursing diagnoses were the risk of violent behavior, sensory perception disturbances (visual hallucinations), ineffective denial, ineffective health management, and ineffective coping. The nursing action plan that will be carried out is the Implementation Strategy Risk of Violent Behavior 1-4 as the main issue raised. Nursing implementation is carried out for 4 meetings with clients and families. After 4 meetings, the Implementation Strategy for the client only reached Implementation Strategy 2 and the family reached Implementation Strategy 4. The evaluation results found that there were positive changes during the meeting with the client. Nursing care with the risk of violent behavior cannot be completed due to the short period of time and the client's lack of cooperation. This final scientific work is expected to be a reference for treating.
