Abstract
Kemoterapi dapat memberikan dampak negatif seperti kekhawatiran, kecemasan, serta rasa takut akibat pengalaman penyuntikan obat yang memberikan rasa tidak nyaman. Salah satu cara untuk mengurangi kecemasan anak selama kemoterapi adalah dengan terapi bermain melalui hand puppet. Tujuan penelitian ini adalah mengetahui efektivitas terapi hand puppet untuk menurunkan kecemasan pada anak yang menjalani kemoterapi di Rumah Sakit Saiful Anwar Malang. Penelitian ini merupakan case study yang melibatkan pasien yang menjalani kemoterapi. Jumlah sampel yang digunakan adalah 6 pasien yang dikelompokkan menjadi dua kelompok yaitu hand puppet dan distraksi napas dalam. Kriteria penentuan sampel adalah bersedia menjadi responden, menjalani kemoterapi, berusia 10-18 tahun, tidak menjalani prosedur operasi, dan pasien dalam keadaan sadar dan dapat berkomuikasi 2 arah. Instrumen yang digunakan adalah Hospital Anxiety and Depression Scale. Hasil penelitian ini menunjukkan bahwa sebelum dilakukan terapi napas dalam rata-rata tingkat kecemasan anak adalah 14±2,64 (kecemasan sedang) dan setelah dilakukan terapi napas dalam rata-rata kecemasan anak mengalami penurunan menjadi 12,3±2,88 (kecemasan sedang). Sebelum dilakukan intervensi hand puppet, rata-rata kecemasan anak adalah 16±2,28 (kecemasan berat) dan setelah dilakukan intervensi hand puppet, rata-rata kecemasan anak mengalami penurunan menjadi 9,3±1,22 (kecemasan ringan). Penggunaan hand puppet dapat menurunkan kecemasan yang lebih besar dibandingkan dengan distraksi pernapasan dengan presentase 73,5. Terapi bermain dengan hand puppet merupakan terapi yang dapat mengeksternalisasi masalah dan menurunkan kecemasan pada anak saat akan menjalani kemoterapi. Dengan demikian, ruangan maupun layanan kesehatan dapat menggunakan terapi hand puppet untuk mengatasi kecemasan anak selama hospitalisasi maupun prosedur invasive.
English Abstract
Chemotherapy can have negative effects such as worry, anxiety, and fear due to the experience of injecting drugs that cause discomfort. One of the ways to reduce children's anxiety during chemotherapy is play therapy through hand puppets. The purpose of this study was to determine the effectiveness of hand puppet therapy to reduce anxiety in children undergoing chemotherapy at Saiful Anwar Hospital Malang. This research is a case study involving patients undergoing chemotherapy. The number of samples used were 6 patients who were grouped into two groups, namely hand puppets and deep breathing distraction. The criteria for determining the sample were willing to be a respondent, undergoing chemotherapy, aged 10-18 years, not undergoing a surgical procedure, and the patient was conscious and able to communicate in two directions. The instrument used is the Hospital Anxiety and Depression Scale. The results of this study showed that before deep breathing therapy the average child's anxiety level was 14 ± 2.64 (moderate anxiety) and after deep breathing therapy the average child's anxiety decreased to 12.3 ± 2.88 (moderate anxiety). Before the hand puppet intervention, the average child's anxiety was 16 ± 2.28 (heavy anxiety) and after the hand puppet intervention, the child's average anxiety decreased to 9.3 ± 1.22 (mild anxiety). The use of hand puppets can reduce anxiety which is greater than breathing distraction with a percentage of 73.5%. Play therapy with hand puppets is a therapy that can externalize problems and reduce anxiety in children when they are going to undergo chemotherapy. Thus, rooms and health services can use hand puppet therapy to overcome children's anxiety during hospitalization and invasive procedures.
