Abstract

Paparan logam berat mampu mengganggu fungsi-sistem reproduksi' wanita dan menjadi salah satu faktor penyebab terjadinya infertilitas. Bahan kimia berbahaya salah satunya adalah logam-berat :aluminium.. Homeostasis besi (Fe).intrasetuler menjadi terganggu dan memodulasi reaksi fenton. Sistem pertahanan enzim antioksidan endogen seperti. Superoksida - Dismutase' (SOD) akan_mengalami 'penurunan, -mengakibatkan akumulasi Reactive. Oxygen -Species (ROS). dan. menyebabkan stres - oksidatif. Peningkatan stres oksidatif menyebabkan peroksidasi lipid dan menghasilkan produk akhir Malondialdehyde (MDA). “Hal 'ini’ dapat ‘mengganggu kerja “ Gonadotropin “Releasing Hormone (GnRH) pada’hipotalamus, mengakibatkan kegagalan hipofisis,untuk melakukan sintesis dan sekresi Follicle Stimulating Hormone (FSH) & Luteinizing Hormone (LH). Penurunan sekresi FSH dan LH'mengakibatkan sekresi hormon ‘estradiol’ (E2) terhambat melalui' 'HPO-axis; sehingga | mengganggu | folikulogenesis: dan <ovulasi yang dapat menyebabkan infertilitas. Pemberian antioksidan eksogen: dapat memperbaiki efek buruk dari paparan” aluminium. Kunyit (Curcuma longa) telah dilaporkan berpotensi sebagai penghambat ” pembentukan''ROS dan merangsang | pembentukan ' enzim' ‘antioksidan endogen- karena :memiliki- aktivitas . antioksidan.yang. kuat. Studi. ini. bertujuan untuk mengetahui pengaruh ekstrak kunyit pada kadar MDA otak dan kadar E2 serum tikus putih betina yang dipapar aluminium. Studi- rini - menggunakan . jenis penelitian: -eksperimental-. laboratorik . .dengan rancangan post test only control group design dan menggunakan sebanyak 25 ekor tikus betina-galur wistar. Tikus ‘dibagi dalam 5 kelompok pengamatan, yaitu:' kontrol negatif (tanpa aluminium dan ekstrak kunyit), kontrol positif (aluminium 100:mg/kgBB), perlakuan 1/P1 (aluminium 100 mg/kgBB+ekstrak kunyit 100 mg/kgBB), perlakuan 2/P2 (aluminium 100 mg/kgBB+ekstrak kunyit 200 mg/kgBB), dan 'perlakuan 3/P3" (aluminium 100 mg/kgBB+ekstrak kunyit 400'mg/kgBB). Ekstrak kunyit dan aluminium diberikan per oral dengan sonde .selama; 30 hari untuk memperoleh -paparan ;subkronis .pada tikus.dan diterminasi pada fase estrus. Pengukuran kadar MDA otak dan E2 serum menggunakan metode 'ELISA (Enzyme Linked Immunosorbent Assay).'Hasil” pengamatan 'dianalisis dengan One - Way Anova atau Kruskal-Wallis. Hasil uji Kruskall-Wallis pada kadar MDA otak terdapat perbedaan bermakna p- value=0,005(a=0.05) pada kelompok sampel. Terdapat perbedaan tidak bermakna antara kelompok yang dipapar aluminium (101,53+44,59) dengan kelompok normal (120,12+27); Namun, secara kuantitas terdapat peningkatan pada Kontrol positif. Rerata kadar MDA pada P2 (54,47 +17,9) dan P3 (44,22 1 24,8) terdapat perbedaan yang bermakna bila dibandingkan dengan kontrol positif. Uji One Way Anova pada kadar E2 serum didapatkan p-value=0,004 (a=0.05)..Paparan aluminium dapat:menurunkan kadar E2 dan pemberian ekstrak kunyit dosis 100 mg/kgBB mampu mencegah penurunan kadar E2 secara signifikan. Kunyit secara tradisional digunakan di negara-negara Asia sebagai ramuan medis karena memiliki aktivitas antioksidan. Kurkuminoid mengacu pada sekelompok senyawa fenolik-yang ‘ada dalam'kunyit, yang diduga menjadi donor elektron hidrogen untuk radikal bebas yang:tidak :stabil. ' Kurkumin-mampu: meningkatkan| kapasitas antioksidan tubuh dengan bertindak. sebagai radical scavenger untuk ROS dan menghambat peroksidasi lipid. Peroksidasi lipid dianggap sebagai mekanisme molekuler utama yang terlibat dalam kerusakan ' oksidatif pada ‘struktur sel akibat produksi ROS! yang-dapat menyebabkan kematian sel. Exposure to heavy metals exposure can ‘disrupt the function’ of the female reproductive: system-and ‘one/ of the factors causing infertility: One of the hazardous chemical is heavy metal-aluminium. Intracellular iron (Fe) homeostasis to be disturbed and modulates the Fenton reaction. Endogenous enzyme antioxidant defense system such-as Superoxide Dismutase (SOD) will decrease, causes the accumulation of Reactive Oxygen Species (ROS)- and . causes, oxidative stress, Increased oxidative stress causes lipid peroxidation causes lipid peroxidation and results in the final product Malondialdehyde (MDA): These can interfere with the 'work of Gonadotropin Releasing Hormone (GnRH) in the hypothalamus, causes pituitary failure to synthesize and,secrete Follicle Stimulating Hormone (FSH) & Luteinizing Hormone (LH). Diminished secretion of FSH & LH causes the 'secretion of the hormone ‘estradiol (E2) to be inhibited through the HPO-axis thereby disrupting: folliculogenesis jand ~ovulation which- can result: iin-infertility. Administered exogenous antioxidants can ameliorate-the adverse effects of aluminum exposure. Turmeric (Curcuma longa) has been reported as potential inhibitor of the formation of ROS and stimulate the formation of .endogenous antioxidant enzymes 'because it has strong antioxidant activity. This study, aims.to determine the effect of turmeric extract on brain MDA levels and serum E2 levels of female white rats exposed to aluminum. This'study was a laboratory experimental study with' a post-test-only control group design and used 25 female wistar rats. Rats were divided into 5 treatment groups, such as: the negative control group (without aluminum and turmeric extract), the positive control group (aluminum 100 mg/kgBW), treatment group 1/P1 (aluminum+turmeric extract 100 mg/kgBW); treatment group. 2-/P2 (aluminum+turmeric_extract: 2001 mg/kgBW), -and treatment group 3/P3 (aluminum+turmeric extract 400 mg/kgBW). Turmeric-extracts and aluminum were given orally with a sonde for 30 days to obtain subchronic exposure to rats and terminated-in the estrous phase. Measurement of brain! MDA levels and serum E2 levels used the ELISA (Enzyme Linked Immunosorbent Assay) method. The observations were analyzed by One Way Anova/Kruskal-Wallis test. The results of the Kruskall-Wallis test on-brain MDA 'levels showed- a significant difference \in- p-value=0.005 . (0=0.05) inthe-samplegroup: There was no-significant difference between the group exposed to aluminum (101.53 z 44.59) and the normal group (120.12 z 27). ' However, interms of quantity there'was an increase in the positive control. There was a significant difference in the mean MDA levels at P2 (54.47-+/17.9):and P3 (44.22 1 24.8) when compared to the positive controls. One Way Anova teston serum E2 levels obtained p-value=0.004 (a=0.05). Exposure to aluminum can reduce £2 levels and giving a dose of 100-mg/kgBB turmeric extract can prevent a significant decrease in E2 levels. Turmeric has been traditionally used in Asian countries as a medical herb due to have antioxidant activity. Curcuminoids refer to a group of phenolic compounds present in turmeric,-which-are thought to be hydrogen electron donors for. unstable free radicals. Curcumin can increase, the antioxidant capacity of the body by act as radical scavengers for ROS and ' inhibit" lipid-peroxidation. Lipid peroxidation is ‘considered -as"the main molecular, mechanism involved in oxidative damage to cell 'structures'due to ROS levels which can lead to demise of the cell. ROS inhibition with antioxidants was able to reduce MDA levels that increases GnRH production, so'it can stimulate the ovaries and improve fertility. The balance between oxidation. and antioxidation. is believed to be critical in maintaining healthy biological systems. However, curcumin has a paradoxical effect, curcumin converts the antioxidant effect into'a pro-oxidant depending on the concentration and ‘cellular environment.
