Hubungan Tingkat Keparahan Diabetic Peripheral Neuropathy Dengan Ketebalan Retinal Nerve Fiber Layer Pada Pasien Diabetes Melitus Tipe 2

Abstract

Tujuan : mengetahui hubungan antara tingkat keparahan Diabetic Peripheral Neuropathy dan ketebalan Retinal Nerve Fiber Layer. Metode : Penelitian dengan desain cross sectional yang di lakukan pada bulan September 2021 – Oktober 2022 di poli Endokrin dan poli Mata RSUD dr Saiful Anwar Malang. Melibatkan pada 18 pasien diabetes melitus (DM) tipe 2 dengan komplikasi diabetic peripheral neuropathy (DPN) dan 18 orang sebagai kelompok DM tipe 2 tanpa komplikasi DPN dengan rentang usia 40-60 tahun, dan tajam penglihatan dengan koreksi lebih baik dari 0,2 logMAR. Dilakukan pemeriksaan pemeriksaan Optical Coherence Tomography (OCT) untuk mengetahui ketebalan Retinal Nerve Fiber Layer (RNFL) dan di lakukan dan pemeriksaan electroneuromyography untuk menegakkan diagnosis DPN. Data dianalisa dengan SPSS versi 23. Hasil : Ketebalan RNFL secara average pada kelompok DM dengan DPN lebih tipis 100.22±38 dibandingkan ketebalan RNFL pada kelompok DM tanpa DPN yaitu sebesar 102.61±9.11 (p 0.444). Kuadran temporal 71.78±12.21 dan nasal 75.11±11.38 lebih rendah dibanding kelompok DM tanpa DPN yaitu 76.33±8.53 pada temporal, 77.39±14 (p 0,203; 0,596). Kesimpulan : Terdapat penurunan ketebalan pada average, temporal dan nasal RNFL pada kelompok DM dengan DPN dibandingkan dengan kelompok DM tanpa DPN walaupun tidak signifikan secara statistik. Terdapat korelasi negatif antara tingkat keparahan DPN dengan Ketebalan RNFL namun hal ini tidak signifikan secara statistik.

English Abstract

Objective: To study the correlation between the severity of diabetic peripheral neuropathy and retinal nerve fiber layer thickness Methods: a cross-sectional study was conducted from September 2021 - October 2022 at the Endocrine and Eye Outpatient Clinic of Dr. Saiful Anwar Hospital, Malang. 36 people were enrolled in this study which is divided into two groups. 18 people with type 2 diabetes mellitus (DM) with complications of diabetic peripheral neuropathy (DPN) and 18 people with type 2 diabetes mellitus without DPN complications. All subjects were 40-60 years old, and the best corrected visual acuity was better than 0,2 logMAR. An Optical Coherence Tomography (OCT) examination was carried out to determine the Retinal Nerve Fiber Layer (RNFL) thickness, and an electroneuromyography examination was carried out to establish a diagnosis of DPN. Data were analyzed with SPSS 23rd version. Results: The average RNFL thickness in the DM group with DPN was 100.22 ± 38 thinner than the RNFL thickness in the DM without DPN group, which was 102.61 ± 9.11 (p 0.444). On the temporal quadrant, 71.78 ± 12.21 and nasal quadrant, 75.11 ± 11.38 were also thinner than DM without DPN group, namely 76.33 ± 8.53 in temporal, 77.39 ± 14 (p 0.203; 0.596). Conclusion: The RNFL thinning of the average, temporal, and nasal quadrant in the DM group with DPN although it was not statistically significant. RNFL thinning has negative correlation with the severity of the DPN but it was not statistically significant.
