Abstract
Preeklampsia berhubungan dengan kegagalan invasi trofoblas dan maladaptasi respon imun. Maladaptasi respon imun akan mengakibatkan ketidakseimbangan sel T helper (Th) tipe 1 dan tipe 2 dalam upaya mempertahankan kehamilan. Statin dapat mengatur produksi sitokin antiinflamasi dan menurunkan produksi sitokin proinflamasi. Efek imunomodulator, anti- inflamasi bersama efek pleiotropik pada pembentukan ROS dan proliferasi sel otot polos, menjadikan pravastatin sebagai obat pilihan yang menjanjikan dalam upaya mencegah dan mengobati preeklampsia. Tujuan penelitian ini adalah membuktikan hubungan antara pemberian pravastatin dengan ekspresi TNF-α dan IL-10 pada ginjal tikus preeklampsia (Rattus norvegius). Penelitian ini menggunakan desain true experimental study dengan post test only with control group design, dilakukan secara in vivo menggunakan bahan biologis tersimpan dari jaringan ginjal tikus bunting model preeklampsia galur Wistar yang dipapar L-NAME dan diberi pravastatin berbagai dosis. Uji saphiro wilk data terdistribusi secara normal p>0.05. Uji One Way Annova pada ekspresi TNF-α dan IL-10 terdapat perbedaan bermakna pada lima kelompok pengamatan (p=0.037; p=0.000). Uji korelasi Pearson yang dilakukan terhadap dosis pravastatin dengan ekspresi TNF-α (r=-.424*,p=0.035); dosis pravastatin dengan ekspresi IL-10 (r=.593**,p=0.002). Berdasarkan hasil uji korelasi disimpulkan bahwa semakin tinggi dosis pravastatin maka dapat menurunkan ekspresi TNF-α dan dapat meningkatkan ekspresi IL-10. Kesimpulan adalah Pravastatin dapat menurunkan ekspresi TNF-α serta dapat meningkatkan ekspresi IL-10 pada ginjal tikus wistar (Rattus norvegicus) Model Preeklampsia.
English Abstract
Preeclampsia is associated with failure of trophoblastic invasion and maladaptation of the immune response. Maladaptation of the immune response will result in an imbalance of type 1 and type 2 T helper (Th) cells in an effort to maintain pregnancy. Statins can regulate the production of anti-inflammatory cytokines and reduce the production of pro-inflammatory cytokines. Immunomodulatory and anti-inflammatory effects along with pleiotropic effects on ROS formation and smooth muscle cell proliferation, make pravastatin a promising drug of choice in efforts to prevent and treat preeclampsia. The purpose of this study was to prove the relationship between pravastatin administration and the expression of TNF-α and IL-10 in the kidneys of preeclamptic rats (Rattus norvegius). This study was a true experiment with a post test only with control group design, carried out in vivo using stored biological material from kidney tissue of preeclamptic rats exposed to L-NAME and given various doses of pravastatin. Shapiro wilk test data normally distributed p>0.05. The One Way Annova test on the expression of TNF-α and IL-10 showed significant differences in the five observation groups (p=0.037; p=0.000). Pearson's correlation test was performed on pravastatin doses with TNF-α expression (r=-.424*, p=0.035); pravastatin dose with IL-10 expression (r=.593**,p=0.002). Based on the results of the correlation test, it was concluded that the higher the dose of pravastatin, the lower the expression of TNF-α and the higher the expression of IL-10. The conclusion is Pravastatin can reduce TNF-α expression and can increase IL-10 expression in the kidneys of Wistar rats (Rattus norvegicus) Preeclampsia model.
