Abstract
Glibenclamide merupakan terapi lini kedua dalam pengobatan Diabetes Mellitus Tipe 2. Glibenclamide memiliki kelarutan rendah di dalam air sedangkan permeabilitasnya tinggi, termasuk ke dalam Biopharmaceutics Classification System kelas 2. Oleh karena itu, bioavailabailitas yang dimilikinya dapat kurang baik karena solubilitasnya yang rendah tersebut. Perbaikan solubilitas dari Glibenclamide dilakukan dengan cara menggunakan sistem penghantaran obat, yakni solid lipid nanoparticles, sehingga bioavailabilitasnya dapat meningkat. Untuk mempelajari bagaimana pengaruh sistem penghantaran obat solid lipid nanoparticles dalam meningkatkan bioavailabilitas obat Glibenclamide, maka dilakukan penelitian deskriptif berupa systematic literature review. Review artikel terstuktur ini dilakukan dengan menggunakan aplikasi Publish or Perish pada tiga database antara lain, Google Scholar, Crossref, dan Scopus. Seleksi dilakukan pada daftar artikel yang didapatkan berdasarkan duplikasi, kriteria inklusi/eksklusi yang telah dibuat, dan critical appraisal yang telah ditentukan. Penyeleksian tersebut juga digambarkan dalam diagram alir PRISMA. Dari database didapatkan sejumlah 1226 artikel, namun hanya 3 artikel yang dapat diterima setelah melalui seleksi yang telah disebutkan. 3 artikel tersebut digunakan sebagai perbandingan dan menjawab rumusan masalah yang ada. Ketiga penelitian menggunakan tikus sebagai hewan cobanya, namun formulasi solid lipid nanoparticles pada masing-masing penelitian berbeda. Walaupun formulasi berbeda dikarenakan banyaknya jenis bahan bertujuan sama, akan tetapi ketiganya dapat menggambarkan sistem penghantaran obat berupa partikel lipid padat berukuran 50 – 1000 nm yang distabilkan oleh surfaktan, fosfolipid, polimer, ataupun kombinasi antara ketiganya tersebut. Kesimpulan dari penelitian ini adalah solid lipid nanoparticles memberikan efek signifikan dalam meningkatkan bioavailabilitas Glibenclamide berdasarkan kadar serum glukosa darah yang diukur di dalam masing-masing penelitian.
English Abstract
Glibenclamide is the second-line therapy in the treatment of Type 2 Diabetes Mellitus. Glibenclamide has low solubility in water while high permeability, it is included in the class 2 of Biopharmaceutics Classification System. Therefore, its bioavailability can be poor due to its low solubility. Improvement of the solubility of Glibenclamide is carried out by using a drug delivery system, namely solid lipid nanoparticles, so that its bioavailability can be increased. To study how the influence of solid lipid nanoparticles drug delivery systems in increasing the bioavailability of Glibenclamide, a descriptive study was conducted in the form of a systematic literature review. This structured article review was carried out using the Publish or Perish application on three databases, namely, Google Scholar, Crossref, and Scopus. Selection is made on the list of articles obtained based on duplication, inclusion/exclusion criteria that have been made, and predetermined critical appraisal. The selection is also described in the PRISMA flowchart. From the databases, a total of 1226 articles were obtained, but only 3 articles were accepted after going through the aforementioned selection. Those three can be used as a comparison and answer the existing problem formulation. The three studies used rats as experimental animals, but the formulation of solid lipid nanoparticles in each study was different. Even though the formulations are different because there are many types of ingredients with the same purpose, all three can describe drug delivery systems in the form of solid lipid particles measuring 50 – 1000 nm stabilized by surfactants, phospholipids, polymers, or a combination of these activities. The conclusion of this research stated that solid lipid nanoparticles had a significant effect in increasing the bioavailability of Glibenclamide based on serum blood glucose levels measured in each study.
