Abstract

Latar Belakang: Kanker nasofaring (KNF) adalah kanker tersering di antara kanker epitel lain di regio kepala leher. Penyebab utama KNF diantaranya adalah infeksi Epstein Barr Virus (EBV), genetik, dan faktor lingkungan. Pembuluh darah hasil dari angiogenesis pada keganasan bersifat abnormal, memiliki peningkatan potensi angiogenesis. Untuk merefleksikan angiogenesis keganasan, maka dibentuk suatu metode kuantifikasi berupa microvessel density (MVD). Vascular Endothelial Growth Factor (VEGF) yang meregulasi angiogenesis diduga berkorelasi dengan MVD. Tujuan: Menganalisis adanya korelasi antara ekspresi VEGF dengan MVD terhadap stadium Karsinoma Nasofaring Tipe Undifferentiated. Metode: Penelitian observasional analitik dengan desain potong-lintang yang melibatkan 28 sampel biopsi jaringan nasofaring dari penderita KNF Tipe undifferentiated. Pemeriksaan ekspresi VEGF dan CD-105 (marker MVD) menggunakan pewarnaan imunohistokimia yang dinilai oleh ahli Patologi Anatomi. Hasil: Analisis statistik ekspresi VEGF dan CD-105 menunjukkan korelasi positif signifikan (p=0,02) dengan koefisien korelasi ρ = 0,683; ekspresi VEGF dan ukuran tumor primer berkorelasi positif signifikan (p=0,01) dengan koefisien korelasi r =0,613; ekspresi VEGF dan ukuran kelenjar getah bening (KGB) leher berkorelasi positif yang tidak signifikan (p=0.65) dengan koefisien korelasi r = 0.089, ekspresi CD-105 dan ukuran tumor primer berkorelasi positif yang tidak signifikan (p=0,06) dengan koefisien korelasi r =0,362, ekspresi CD-105 dan ukuran KGB leher berkorelasi positif yang tidak signifikan (p=0,33) dengan koefisien korelasi r =0,191. Ekspresi VEGF dan stadium KNF berkorelasi positif yang signifikan (p=0,01) dengan koefisien korelasi r =0,730, Ekspresi VEGF dan stadium KNF berkorelasi positif yang signifikan (p=0,01) dengan koefisien korelasi r =0,836. Kesimpulan: Semakin tinggi ekspresi VEGF diikuti meningkatnya ekspresi CD-105 (p=0,1) pada jaringan nasofaring penderita KNF tipe undifferentiated. Semakin tinggi ekspresi VEGF dan CD-105 diikuti meningkatnya stadium KNF.

English Abstract

Background: Nasopharyngeal cancer (NPC) is the most common cancer among other epithelial cancers in the head and neck region. The main causes of NPC include Epstein Barr Virus (EBV) infection, genetics, and environmental factors. The blood vessels resulting from angiogenesis in malignancy are abnormal, having an increased angiogenic potential. To reflect the angiogenesis of malignancy, a quantification method in the form of microvessel density (MVD) was developed. Vascular Endothelial Growth Factor (VEGF) which regulates angiogenesis is thought to correlate with MVD. Objective: Analyzing the correlation of VEGF expression with MVD in the Undifferentiated Type of Nasopharyngeal Carcinoma stage. Method: An analytic observational study with a cross-sectional design involving 28 samples of nasopharyngeal tissue biopsies from patients with undifferentiated NPC. Examination of VEGF and CD-105 (MVD marker) expression using immunohistochemical staining assessed by an Anatomical Pathologist. Result: Statistical analysis of VEGF and CD-105 expression showed a significant positive correlation (p=0.02) with a correlation coefficient ρ = 0.683; VEGF expression and primary tumor size had a significant positive correlation (p=0.01) with a correlation coefficient r=0.613; VEGF expression and cervical lymph node size had a non-significant positive correlation (p=0.65) with a correlation coefficient of r = 0.089, CD-105 expression and primary tumor size had an insignificant positive correlation (p=0.06) with a correlation coefficient of r =0.362 , CD-105 expression and cervical lymph node size had a positive but insignificant correlation (p=0.33) with a correlation coefficient r=0.191. VEGF expression and NPC stage had a significant positive correlation (p=0.01) with a correlation coefficient of r =0.730, VEGF expression and NPC stage had a significant positive correlation (p=0.01) with a correlation coefficient of r =0.836. Conclusion: Higher VEGF expression was followed by increased CD-105 expression (p=0.1) in the nasopharyngeal tissue of patients with undifferentiated NPC. The higher expression of VEGF and CD-105 was followed by an increase in the NPC stage.
