Abstract
Pendahuluan: Hipovitaminosis D pada LES memperparah aktivitas penyakit. Suplementasi vitamin D telah terbukti dapat mengurangi keparahan LES, meskipun hasilnya belum memuaskan. Untuk meningkatkan efek vitamin D, ditambahkan kurkumin, sebagai ligan alami reseptor vitamin D. Untuk meningkatkan absorpsi kurkumin, diberikan kombinasi dengan piperine. Penelitian ini meneliti efek suplementasi vitamin D dan kurkumin-piperine, terhadap skor Fatigue Severity Scale (FSS) dan TGF-β pada pasien LES. Metode: Penelitian ini merupakan uji ganda acak terkontrol, yang dilakukan dalam waktu tiga bulan pada 45 pasien LES. Subjek penelitian dibagi menjadi tiga grup suplementasi: Grup 1 (Vitamin D3 3x400 IU dan plasebo 3x1 kapsul, n=15), Grup 2 (Kurkumin-Piperine 600 mg/15.800 mg 1x1 kapsul, dan plasebo 3x1 kapsul, n=15), dan Grup 3 (Vitamin D3 3x400 IU dan Kurkumin-Piperine 600 mg/15.800 mg 1x1 kapsul, n=15). Semua subjek dinilai menggunakan Skor FSS dan kadar TGF-β serum pada awal dan 3 bulan setelah pengobatan. Hasil: Tidak didapatkan peningkatan signifikan kadar TGF-β sebelum dan sesudah perlakuan pada grup Vitamin D (p=0,53) dan Kurkumin-piperine tunggal (p=0,078), peningkatan bermakna didapatkan pada grup Vitamin D + Kurkumin-piperine (p=0,001). Penurunan yang signifikan skor FSS pada semua grup baik pada grup Vitamin D (p=0,002), kurkumin-piperine (p=0,001), dan vitamin D + kurkumin-piperine (p=0,001). Perubahan TGF-β dan FSS secara signifikan lebih tinggi pada grup Vitamin D + Kurkumin-piperine dibandingkan dengan Grup Vitamin D atau Kurumin-piperine tunggal (p=0,005 dan p<0,001). Kesimpulan: Kadar TGF-β dan skor FSS pada pasien LES mengalami perbaikan dengan suplementasi vitamin D atau kurkumin-piperine saja, tetapi kombinasi vitamin D dan kurkumin-piperine memberikan hasil yang terbaik.
English Abstract
Introduction: Hypovitaminosis D in SLE aggravates disease activity. Vitamin D supplementation has been shown to reduce SLE severity, although the results were unsatisfying. To increase the effect of vitamin D, curcumin is added, as natural ligand for the vitamin D receptor. To increase curcumin absorption, combination with piperine is added. This study examined the effects of Vitamin D and curcumin-piperine supplementation, on the TGF-β and Fatigue Severity Scale (FSS) score and in SLE patients Methods: This study was double-blind randomized controlled trial conducted within three months to 45 SLE patients. Subjects were divided into three groups: Group 1 (Vitamin D3 3x400 IU and placebo 3x1, n=15), Group 2 (Curcumin-Piperine 600 mg/15.800 mg 1x1, and placebo 3x1, n=15), and Group 3 (Vitamin D3 3x400 IU and Curcumin-Piperine 600 mg/15.800 mg 1x1, n=15). All participants were assessed using FSS and TGF-β at baseline and 3 months after treatment. Results: There was no significant increase TGF-β before and after treatment in Vitamin D group (p=0.53) and Curcumin-piperine alone (p=0.078), significant increase was obtained in Vitamin D + Curcumin-piperine group (p=0.001). There was significant decrease FSS in all groups in Vitamin D (p=0.002), curcumin-piperine (p=0.001), and vitamin D + curcumin-piperine (p=0.001). The changes in TGF-β and FSS were significantly higher in Vitamin D + Curcumin-piperine group compared to Vitamin D or Curcumin-piperine group (p=0.005 and p<0.001, respectively). Conclusion: TGF-β levels and FSS score in SLE patients can be improved by vitamin D or curcumin-piperine alone, but the combination vitamin D and curcumin-piperine produced the best outcomes.
