Abstract

Antibiotik golongan beta-laktam merupakan golongan antibiotik yang paling umum menyebabkan reaksi hipersensitivitas. Hipersensitivitas akibat antibiotik dapat menimbulkan kerugian ekonomi, waktu, dan terapi pada pasien. Penelitian ini bertujuan untuk mengetahui faktor risiko hipersensitivitas akibat paparan antibiotik golongan beta-laktam melalui penelitian-penelitian yang sudah dilakukan sebelumnya dengan metode systematic literature review. Pencarian literatur dilakukan menggunakan aplikasi Publish or Perish pada tiga database (Google Scholar, PubMed, dan Scopus) dengan kata kunci “Beta Lactam Antibiotic AND Hypersensitivity OR Allergy.” serta pencarian manual di Google Scholar dengan kata kunci “Faktor risiko hipersensitivitas antibiotik beta-laktam”. Artikel kemudian diseleksi menggunakan protokol Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA). Artikel selanjutnya dihimpun dan diseleksi berdasarkan kriteria pemilihan dan PICO. Artikel dinilai menggunakan critical appraisal tools dari Joanna Briggs Institute sesuai dengan desain penelitian masing-masing artikel. Hasil seleksi mendapatkan 18 artikel yang memenuhi tahapan eligibility pada diagram alir PRISMA karena memenuhi kriteria pemilihan, PICO, dan kriteria critical appraisal sehingga dianggap layak (eligible) untuk dianalisis dalam penelitian. Artikel tersebut terdiri dari dua artikel dengan desain penelitian true experimental, tiga artikel quasi-experimental, satu artikel case�report, lima artikel penelitian case-control, lima artikel penelitian cross-sectional, dan dua artikel penelitian kohort. . Berdasarkan hasil penelitian, dapat disimpulkan bahwa faktor risiko pemicu hipersensitivitas baik tipe I, II, III, maupun IV akibat antibiotik golongan beta-laktam adalah faktor genetika, reaktivitas silang, riwayat penyakit tertentu, serta perbedaan rute, frekuensi, dan durasi pemberian antibiotik golongan beta-laktam. Beberapa agen beta-laktam dan produk degradasinya juga dapat memiliki mekanisme tertentu yang dapat memicu hipersensitivitas.

English Abstract

Beta-lactam antibiotic is the most common class of antibiotics that cause hypersensitivity reactions. Antibiotic-induced hypersensitivity can cause economic, time, and therapeutic disadvantages in patients. This study aims to determine the risk factors of the occurrence of beta-lactam exposure-induced hypersensitivity through existing studies using a systematic literature review method. A literature search was performed with the help of Publish or Perish application on Google Scholar, PubMed, and Scopus database. The keywords used were “Beta Lactam Antibiotic AND Hypersensitivity OR Allergy.” National articles were also explored manually in Google Scholar using keywords “Faktor risiko hipersensitivitas akibat antibiotik beta-laktam”. Articles were then selected using the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) protocol. Articles were then compiled and selected based on selection criteria and PICO. Subsequent articles were assessed using critical appraisal tools from the Joanna Briggs Institute according to each’s research design. The selection results obtained 18 articles that were stated as eligible because met the selection criteria, PICO, and critical appraisal criteria and thus can be included in further analysis in this research. These articles have six different research designs: two true experimental articles, three quasi-experimental articles, one case report, five case-control studies, five cross-sectional studies, and two cohort studies. Based on the result, the triggering factors of type I, II, III, and IV beta-lactam-induced hypersensitivity are genetic factors, cross-reactivity, history of certain diseases, and routes, frequency, and duration of administration variation of beta-lactam antibiotics. Some beta-lactam agents also have certain mechanisms that may induce hypersensitivity.
