Abstract

Seorang wanita akan melalui fase menyusui segera setelah melahirkan. Namun, tidak semua ibu langsung mendapati ASInya keluar pada kondisi post partum. Pada neonatus dengan kelahiran prematur biasanya bayi menyusu lebih sedikit diawal kelahiran daripada bayi yang lahir dengan cukup bulan. Hal ini dikarenakan belum matangnya keadaan fisiologis dan patologis bayi baru lahir. Selain itu, kelahiran prematur menyebabkan rusaknya fase laktogenesis II yang berkaitan dengan ketidakcukupan ASI. Salah satu cara yang dapat dilakukan untuk menangani permasalahan produksi ASI pada ibu post partum adalah dengan terapi nonfarmakologis dengan penerapan teknik effleurage massage. Studi kasus ini bertujuan untuk mengetahui pengaruh dari penerapan teknik effleurage massage terhadap produksi ASI pada ibu post partum dengan kelahiran bayi prematur dan BBLR spontan pervaginam. Karya ilmiah ini menggunakan metode case study dengan memberikan intervensi berupa teknik effleurage massage pada Ny. A yang melahirkan anak pertamanya pada usia kehamilan 34-35 minggu dengan berat badan lahir 2400 gram. Penerapan teknik effleurage massage dilaksanakan selama 4 hari dengan 2 kali intervensi. Hasil pemberian teknik effleurage massage selama 10-15 menit, didapatkan bahwa klien merasa nyaman dan rileks serta terdapat peningkatan volume ASI, peningkatan jumlah BAK bayi, dan penambahan berat badan pada bayi. Pemberian teknik effleurage massage diketahui berpengaruh terhadap produksi ASI. Bagi tenaga kesehatan, dengan adanya karya ilmiah dan studi kasus ini diharapkan dapat menjadi acuan dalam pemberian asuhan keperawatan terkait masalah ketidakefektifan menyusui.

English Abstract

A woman will go through a breastfeeding phase immediately after giving birth. However, not all mothers immediately find their milk coming out in post partum conditions. In neonates with premature births, babies usually suckle less at the beginning of birth than babies born at full term. This is due to the immaturity of the physiological and pathological state of the newborn. In addition, premature birth causes damage to the lactogenesis II phase which is associated with milk insufficiency. One way that can be done to deal with the problem of milk production in post partum mothers is non-pharmacological therapy by applying the effleurage massage technique. This case study aims to determine the effect of applying the effleurage massage technique on breast milk production in post partum mothers with spontaneous vaginal premature and low birth weight babies. This scientific work uses the case study method by providing interventions in the form of effleurage massage techniques to Mrs. A, who gave birth to her first child at 34-35 weeks' gestation with a birth weight of 2400 grams. The application of the effleurage massage technique was carried out for 4 days with 2 interventions. The results of giving the effleurage massage technique for 10-15 minutes, it was found that the client felt comfortable and relaxed and there was an increase in the volume of breast milk, an increase in the number of baby's urination, and weight gain in the baby. Giving effleurage massage technique is known to affect milk production. For health workers, with the presence of scientific work and case studies this is expected to be a reference in providing nursing care regarding the problem of breastfeeding ineffectiveness.
