Abstract

Deep vein thrombosis (DVT) menyebabkan komplikasi emboli paru yg merupakan penyebab kematian ketiga terbanyak akibat penyakit kardiovaskuler. Deteksi dini DVT dengan pemeriksaan fisik, tes Wells score, D-dimer dan ultrasonografi. Insiden DVT pada pasien pre-operasi daerah lutut atau panggul sebesar 2.6- 17.3% yang menyebabkan komplikasi fatal pada pre maupun peri-operasi. Tujuan penelitian ini adalah mengevaluasi kesesuaian kategori Wells score dengan gambaran ultrasonografi dalam menentukan adanya deep vein thrombosis pada pasien pre-operasi daerah sendi lutut atau panggul di RSUD Dr. Saiful Anwar Malang. Penelitian observasional analitik dengan metode pengambilan data secara cross sectional yang dilakukan di RSUD dr. Saiful Anwar Malang pada Mei 2020 hingga Januari 2021. Penelitian ini melibatkan 29 pasien yang terdiri atas 13 laki-laki dan 16 perempuan. Mayoritas pasien (27 orang) memiliki kategori Wells score unlikely yang sesuai hasil ultrasonografi bukan DVT. Terdapat 1 pasien yang memiliki kategori Wells score likely namun dari ultrasonografi bukan DVT. Sedangkan 1 pasien kategori Wells score unlikely namun hasil ultrasonografi DVT. Tidak didapatkan perbedaan kategori Wells score dengan hasil ultrasonografi dengan nilai p=1.000. Dari penelitian ini didapatkan kesesuaian kategori Wells score dengan hasil ultrasonografi pada pasien yang bukan DVT. Selain itu, ditemukan juga bahwa epidemiologi, status gizi, riwayat penyakit, riwayat operasi dan hasil laboratorium D-dimer berpengaruh terhadap terjadinya DVT. Ultrasonografi mempunyai peranan penting untuk deteksi DVT pada pasien resiko tinggi

English Abstract

Deep vein thrombosis (DVT) causes pulmonary embolism, which is the third largest cause of death due to cardiovascular disease. Early detection of DVT employs physical examination, Wells score test, D-dimer and ultrasonography. The incidence of DVT in pre-operative knee or pelvic area patients is 2.6-17.3%, which leads to fatal complications in pre- and peri-operative periode. The objective of this study is to analize the compatibility of Wells score categories with ultrasonography imaging to determine DVT in pre-operative knee or pelvic joint patients at dr. Saiful Anwar Hospital. This is an analytic observational study with a cross-sectional approach conducted at dr. Saiful Anwar Hospital from May 2020 until January 2021. Twenty-nine preoperative patients were involved, consisting of 13 men and 16 women. The majority (27) of patients had an unlikely Wells score category matching with their non-DVT ultrasonography result. There was one patient who had a likely Wells score, albeit having a non-DVT ultrasonography result. However, one patient had an unlikely Wells score, but the ultrasonography result showed DVT. There was no difference found between the Wells score category and the result of ultrasonography, p=1.000. From this study, it was found that Wells score category corresponds to the result of ultrasonography in non-DVT patients. In addition, it was also found that epidemiology, nutritional status, past medical-surgical history, a laboratory results of D-dimer have an effect on the occurrence of DVT. Ultrasonography has an important role in the detection of DVT in high-risk patients
