Abstract
Angka kejadian penyakit jantung bawaan (PJB) di seluruh dunia diperkirakan 1,2 juta kasus dari 135 juta kelahiran setiap tahunnya. Salah satu penanganan tindakan dari PJB yaitu dengan kateterisasi jantung. Kateterisasi jantung membawa resiko komplikasi terkait dengan pelepasan introducer sheat, salah satu komplikasinya adalah hematoma. Hematoma yang besar dapat menyebabkan nyeri dan pseudoaneurysma Penelitian ini bertujuan untuk menganalisis hubungan prosedur pelepasan introducer sheat dengan metode manual terhadap kejadian hematoma pada paska kateterisasi jantung pada pasien PJB di Rumah Sakit dr. Saiful Anwar Malang. Penelitian ini merupakan penelitian cross sectional dan melakukan observasi sebelum dan sesudah pelepasan introducer sheat femoralis paska kateterisasi jantung pada pasien jantung bawaan. Hasil uji Chi Square Fisher Exact didapatkan nilai p sebesar 0,541 (< 0,05) yang artinya tidak ada hubungan yang signifikan prosedur pelepasan introducer sheat femoralis metode manual terhadap kejadian hematoma pada responden penelitian ini. Penekanan lama sesuai SOP pelepasan introducer sheat yaitu sekitar 15- 20 menit akan mengurangi kejadian hematoma. karena teknik penekanan ini akan membuat tubuh mempunyai fungsi hemostasis untuk pembukan darah sehingga cepat tertutupnya luka di daerah tusukan introducer sheat. Kepatuhan SOP prosedur pelepasan introducer sheat femoralis metode manual sangat penting dilakukan untuk menekan kejadian hematoma, sehingga supervisi saat melakukan tindakan pelepasan introducer sheat perlu ditingkatkan agar tidak terjadi komplikasi hematoma yang menyebabkan length of stay memanjang
English Abstract
The incidence of congenital heart disease (CHD) worldwide is estimated at 1.2 million cases out of 135 million births each year. One of the treatments for CHD is cardiac catheterization. Cardiac catheterization carries the risk of complications associated with removing the introducer sheat, one of which is hematoma. Large hematomas can cause pain and pseudoaneurysms. The purpose of this study is to investigate the correlation between the introducer sheath removal procedure and the manual method for the occurrence of hematomas following cardiac catheterization in CHD patients at Dr. Saiful Anwar Malang's office. This study was a cross-sectional study and made observations before and after removal of the femoral sheath introducer after cardiac catheterization in congenital heart patients. The results of the Fisher Exact Chi Square test obtained a p-value of 0.541 (<0.05), which means that there was no significant relationship between the manual femoral sheat introducer sheath removal procedure and the incidence of hematoma in the respondents to this study. Long pressing for the recommended 15-20 minutes, as per the SOP, can lessen the likelihood of developing hematomas when the introducer heat is released. because the introducer sheat puncture area wound will heal quickly thanks to the body's natural hemostatic function that this suppression strategy will cause to activate. SOP Improvements in supervision are required when performing the procedure to remove the introducer sheat in order to prevent complications from hematoma that would lengthen the length of stay. Compliance with the procedure for manually removing the femoral introducer sheat is crucial to reducing the incidence of hematoma.
