Abstract
Latar Belakang: Kanker paru saat ini masih menjadi penyebab kematian akibat keganasan nomor 2 di dunia. Di Indonesia, insidensi kanker paru termasuk dalam tiga besar penyakit kanker. Volatile Organic Compounds (VOC) dari udara pernapasan dapat mencerminkan perubahan metabolism yang disebabkan oleh penyakit dan kemungkinan bisa sebagai biomarker kanker paru. Interleukin-23 (IL-23) dikenal sebagai sitokin pro-inflamasi dalam pertumbuhan dan progresivitas sel kanker. Tujuan penelitian ini untuk mengetahui perbedaan serta menganalisis hubungan kadar IL-23 dengan senyawa VOC pada pasien kanker paru. Metode: Penelitian ini melibatkan 40 pasien kanker paru dan 42 kontrol. VOC diambil melalui udara ekshalasi napas kedalam kantong khusus dan dianalisis dengan breath analyzer. Pemeriksaan IL-23 diambil dari serum darah dan dianalisis menggunakan metode ELISA. Hasil: Senyawa Ozon (p=0.000), Etanol (p=0.000), Formaldehida (p=0.000), Toluena (p=0.000), Aseton (p=0.000), Amonia (p=0.000), Amonium (p=0.001), Nitrogen (p=0.001) dan Metana (p=0.000) memiliki perbedaan yang signifikan antara pasien kanker paru dan kontrol. Konsentrasi IL-23 memiliki perbedaan yang signifikan antara pasien kanker paru dan kontrol (p=0.000). Analisis korelasi Spearman menunjukkan korelasi positif antara serum IL-23 dengan Formaldehida (p=0.029), Toluena (p=0.014) dan Amonia (p=0.028) dan korelasi negatif dengan Nitrogen (p=0,011). Kesimpulan: Terdapat perbedaan kadar Ozon, Etanol, Formaldehida, Toluena, Aseton, Amonia, Amonium, Nitrogen dan Metana pada kanker paru. Kadar IL-23 meningkat pada kanker paru dan terdapat hubungan antara IL-23 dengan kadar senyawa Formaldehida, Toluena, Amonia dan Nitrogen.
English Abstract
Background: Lung Cancer is the second leading cause of death due to malignancy in the world. In Indonesia, lung cancer is one of the top three malignancies. Volatile organic compounds (VOC) from respiratory reflect changes metabolism caused by disease and may be a biomarker of lung cancer. Interleukin-23 (IL-23) has been known as a pro-inflammatory cytokine in the development and progression of cancer. The goal of this study was to identify levels of IL-23 and VOC in lung cancer patients. Methods: This study was carried out recruiting 40 lung cancer patients and 42 controls. VOC are taken through exhale their breath into the sample bag and it was immediately analyzed. IL-23 was taken from blood serum and then it was analyzed by using ELISA method. Results: The concentration of Ozone (p=0.000), Ethanol (p=0.000), Formaldehydes (p=0.000), Toluene (p=0.000), Acetone (p=0.000), Ammonia (p=0.000), Ammonium (p=0.001), Nitrogen (p=0.001) and Methane (p=0.000) were significant differences between lung cancer patients and controls. The concentration of IL-23 was significant differences between lung cancer patients and controls (p=0.000). Spearman correlation analysis revealed a positive correlation between serum IL-23 with Formaldehydes (p=0.029), Toluene (p=0.014) and Ammonia (p=0.028) and a negative correlation with Nitrogen (p=0.011). Conclusion: There are significant differences in parameter compounds of Ozone, Ethanol, Formaldehydes, Toluene, Asetone, Ammonia, Ammonium, Nitrogen and Methane in the lung cancer. There was increase IL-23 levels in the lung cancer patients and there was association between IL-23 and concentration of Formaldehydes, Toluene, Ammonia and Nitrogen.
