Abstract
Kanker payudara merupakan kanker terbanyak nomor satu dengan jumlah kematian terbanyak. Densitas payudara merupakan faktor risiko independen terhadap kejadian kanker payudara. Estrogen dan progesteron merupakan salah satu faktor yang meningkatkan densitas parenkim payudara. Faktor Reproduksi wanita dapat mempengaruhi kadar estrogen dan progesterone. Profil lipid dapat dijadikan indikator untuk evaluasi kadar estrogen. Belum banyak penelitian yang membahas tentang pengaruh faktor reproduksi dan profil lipid terhadap densitas parenkim payudara. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh Faktor Reproduksi Wanita dan Profil Lipid terhadap Densitas Parenkim Payudara. Metode penelitian ini berupa observasional analitik, rancangan studi cross sectional dari data sekunder di RSUD Dr. Saiful Anwar Malang. Penelitian ini melibatkan 34 wanita dengan rentang usia 35-67 tahun. Hasil mamografi menunjukkan densitas parenkim payudara paling banyak adalah tipe C (58,8%) sesuai ACR-BIRADS. Didapatkan pengaruh jumlah paritas terhadap densitas parenkim payudara (p=0,031); RP 5,25; IK95%(1,093-25,211). Tidak didapatkan pengaruh riwayat laktasi (p=0,529); status menopause (p=0,097); Riwayat penggunaan kontrasepsi hormonal (p=0,711), Riwayat terapi hormonal (p=1,000), usia menarke (p=0,897), usia saat pertama kali melahirkan (p=0,549). kadar kolesterol total (p=0,994), LDL (p=0,585), HDL (p=0,691) maupun trigliserida (p=0,564) terhadap densitas parenkim payudara. Terdapat pengaruh jumlah paritas terhadap densitas parenkim payudara. Tidak terdapat pengaruh faktor reproduksi lainnya maupun profil lipid terhadap densitas parenkim payudara.
English Abstract
Breast cancer is the most frequent cancer with the highest number of deaths. Breast density is an independent risk factor for the incidence of breast cancer. Estrogen and progesterone are factors that increase the density of the breast parenchyma. Female reproductive factors can affect estrogen and progesterone levels. Lipid profile can be used as an indicator to evaluate estrogen levels. Not many studies have discussed the effect of reproductive factors and lipid profile on breast parenchymal density. The objective of this study is to determine the effect of female reproductive factors and lipid profile on breast parenchymal density. This is analytical observational study, cross sectional study design from secondary data at RSUD Dr. Saiful Anwar Malang. This study involved 34 women with an age range of 35-67 years. Mammography results showed that the highest density of breast parenchyma was type C (58.8%) according to ACR-BIRADS. Number of parity influence on breast parenchyma density (p=0.031); PR 5.25; CI95%(1,093-25,211). There was no effect of breastfeeding history (p=0,529); menopausal status (p=0.097); History of using hormonal contraception (p=0.711), history of hormonal therapy (p=1,000), age at menarche (p=0.897), age at first birth (p=0.549). cholesterol total (p=0.994), LDL (p=0.585), HDL (p=0.691) and triglycerides (p=0.564) on the density of the breast parenchyma. There is an effect of parity number on breast parenchymal density. There was no influence of other reproductive factors or lipid profile on breast parenchyma density.
