Abstract

Penelitian ini bertujuan untuk mengeksplorasi serta mengungkap proses pengambilan keputusan Manajemen RSUD Chasbullah AbdulMadjid untuk tetap mempertahankan pilihan keputusan pembiayaan Jamkesda Kota Bekasi berdasar fenomena yang terjadi, yaitu fenomena perubahan sasaran layanan Jamkesda, terdapatnya pasien banjir, tekanan permintaan layanan poli VCT dan upaya klaim Covid-19 RSUD CAM, yang sebelumnya tidak tercakup dalam aturan layanan Jamkesda Kota Bekasi. Melalui grounded theory, dioperasionalisasikan konsep proses pengambilan keputusan pada manajemen RSUD CAM Kota Bekasi dengan mengembangkan model pilihan rasional (rational choice) serta eksplorasi proses terjadinya pembentukan rasionalisasi pengambilan keputusan dalam upaya mempertahankan keputusan. Partisipan penelitian terdiri dari Direktur RSUD CAM selaku aktor pengambilan keputusan pada RSUD CAM, Ketua Tim Pelaksana Program Jamkesda pada Dinas Kesehatan Kota Bekasi, serta pengawas program Jamkesda. Pengumpulan data dilakukan melalui observasi, wawancara dan focus group discussion, untuk selanjutnya dilakukan pengkodean. Validasi data dilakukan melalui proses triangulasi serta melakukan proses hubungan antar konsep. Hasil penelitian menunjukkan bahwa optimalisasi pengambilan keputusan pada RSUD CAM untuk tetap mempertahankan pembiayaan Jamkesda Kota Bekasi terbentuk dari faktor interaksi intersubyektivitas, sebagai bentuk rasionalitas kolektif pengambil keputusan internal (manajemen teknis/operasional RSUD CAM) yang diintervensi oleh campur tangan eksternal (manajemen strategis). Karakteristik pengambilan keputusan yang terjadi pada RSUD CAM tersebut mendekati karakteristik model pengambilan keputusan politis yang diberlakukan pada unit birokrasi pemerintahan. Hubungan antar karakteristik pengambilan keputusan yang terjadi dalam satu kategorial tersebut, dalam penelitian ini, selanjutnya termodifikasi menjadi suatu bentuk model pengambilan keputusan birokrasi politis. Sinergitas karakteristik pengambilan keputusan tersebut merupakan stimulus optimalisasi pilihan keputusan manajemen RSUD CAM untuk tetap mempertahankan pembiayaan Jamkesda Kota Bekasi.

English Abstract

The objective of this study is to explore and disclose the process run by the management of Chasbullah AbdulMajid, a local public hospital in Bekasi city, in making the decision of keeping the use of the city’s local health insurance (Jamkesda) funding in response to the recent events such as changes in the insurance’s service target, the presence of flood-related patients, pressures in the supply of VCT service, and the hospital’s claim effort for Covid-19 cases, that were not included in the city’s Jamkesda service regulations. Using grounded theory, a concept of decision-making process was operationalized by the hospital’s management through the development of rational choices and exploration of decision-making rationalization process to maintain their decisions. The research participants are the hospital’s director as the actor of the decision making, the Chief Executive of Jamkesda Program of Bekasi city’s Health Office, and Jamkesda program supervisors. The data was collected through observations, interviews, and group discussions, and then coded afterwards. The data validation was performed through triangulation and inter-conceptual relationship processes. This study finds that the optimization of the hospital’s decision to keep the funding of Bekasi city’s Jamkesda was formed from intersubjectivity interaction factors as a form of internal decision maker’s collective rationality, i.e. the hospital’s technical and/or operational management, with external intervention of strategic management. The characteristics of the decision-making process made by the hospital is similar to that applied by governmental bureaucratic units in their political decisions. The relations between decision making characteristics in that category was then, in this research, modified into a model of political bureaucratic decision making. The synergy of those decision-making characteristics serves as a stimulus for the optimization of the hospital management’s choice to keep the funding of Bekasi city’s Jamkesda.
