Abstract
Latar belakang. Coronavirus Disease 2019 (COVID-19) merupakan penyakit gangguan pernapasan akut disebabkan virus SARS-CoV-2. Kelompok laki-laki, berusia lanjut dengan penyakit penyerta lebih mudah menderita sakit berat akibat COVID-19. Goel et al menjelaskan pasien COVID-19 dengan penyakit penyerta kardiovaskuler berisiko mudah terkena koinfeksi dengan pathogen lain.Tujuan. Menganalisis faktor risiko demografik usia, jenis kelamin dan penyakit penyerta dengan kejadian koinfeksi bakterial pada pasien COVID-19 di Ruang Perawatan Intensif COVID-19 RSUD dr. Saiful Anwar Malang. Metode. Penelitian analitik observasional dengan desain case control study dari pasien COVID-19 yang dirawat periode Oktober 2020-Maret 2021. Data karakteristik demografik, penyakit penyerta dan hasil kultur sputum pasien diambil dari status rekam medik dan laboratorium Mikrobiologi Klinik RSUD Dr.Saiful Anwar Malang dan dianalisis secara statistik menggunakan software SPSS versi 23 dengan uji chi square atau fisher exact. Hasil. Dari total pasien 59 orang, 30 pasien laki-laki (50,8%) dan 29 pasien wanita (49,2%) dengan rerata usia keseluruhan 51,51 ± 14,08 tahun. Bakteri kokus Gram positif yang ditemukan yaitu Staphylococcus aureus 3 isolat (5,1%) dan bakteri batang Gram negatif terbanyak yaitu Acinetobacter baumannii 16 isolat (27,2%), paling sedikit yaitu Enterobacter aerogenes 1 isolat (1,7%). Tidak ada hubungan bermakna antara penyakit penyerta baik diabetes, hipertensi serta karakteristik demografik yaitu usia terhadap hasil kultur sputum. Variabel yang signifikan dengan p<0,05 adalah jenis kelamin. Kesimpulan. Pasien COVID-19 laki-laki berisiko lebih tinggi terkena koinfeksi bakteri disebabkan karena ekspresi ACE2 yang lebih tinggi, merokok, kurang taat terhadap aturan pembatasan sosial selama masa pandem
English Abstract
Introduction. Coronavirus Disease 2019(COVID-19) is an acute respiratory disease caused by SARS-COV-2 virus.Elderly people, men with comorbidities are more likely to suffer from severe illness due to COVID-19. Goel et al.,(2021) explained that patients with comorbidities are at risk of being coinfected with other pathogen. Objective. Analyzing the relationship between demographic risk factor of age, gender and comorbidities with incidence of bacterial coinfection in COVID-19 patients in COVID-19 Intensive Care Unit General hospital Dr.Saiful Anwar Malang. Methods. Observational analytic case control study with patients admitted to the hospital from October 2020-March 2021. Demographic characteristic, comorbidities and sputum culture data taken from medical record and Clinical Microbiology culture. Data will collect and analysis using SPSS Software version 23.00. Results. Of total 59 patients, 30 were male patients (50,8%) and 29 female patients(49,2%) with an overall mean age of 51,51±14,08 years. Gram Positive cocci bacteria were Staphylococcus aureus 3 isolates (5,1%). The most Gram negative rods were Acinetobacter baumannii 16 isolates (27, 2%) and Enterobacter aerogenes 1 isolat (1,7%). There was no significant relationship between co-morbidities such as diabetes, hypertension and age with sputum culture. The significant variable is gender with p<0,05. Conclusion. Male COVID-19 at a higher risk of bacterial coinfection due to higher ACE2 expression, smoking, lack of compliance to social distancing rules during pandemic
