Abstract
Latar belakang dan tujuan: Penyakit ginjal kronis (PGK) atau Chronic Kidney Disease (CKD) merupakan masalah kesehatan yang serius pada anak-anak, dengan meningkatnya morbiditas dan mortalitas. Hubungan antara defisiensi vitamin D dan status inflamasi telah diketahui dengan baik. Vitamin D memiliki peran penting dalam mengurangi beban inflamasi. Beberapa penelitian menunjukkan bahwa rasio neutrofil / limfosit (NLR) merupakan penanda potensial untuk menentukan peradangan. Jadi tujuan dari penelitian ini adalah Mengetahui pengaruh pemberian suplementasi vitamin D3 terhadap kadar 25(OH)D plasma, penurunan nilai Neutrophil to Lymphocyte Ratio (NLR), dan laju filtrasi glomerulus bila dibandingkan dengan plasebo pada anak dengan penyakit ginjal kronis Desain, sampel & pengukuran: Penelitian ini adalah penelitian eksperimental murni dengan rancangan penelitian randomized clinical trial (RCT) double blind, pre and post test control group dengan perlakuan berupa pemberian vitamin D3 oral yang diberikan selama 12 minggu. Kelompok penelitian terdiri dari dua kelompok yaitu kelompok 1 (K1) dan kelompok 2 (K2). Dilakukan uji normalitas data sampel dengan uji Saphiro Wilk. Perbedaan kadar vitamin D, nilai NLR, dan LFG dilakukan uji t berpasangan dan independen. Hubungan kadar vitamin D, nilai NLR dengan LFG dilakukan uji korelasi Pearson. Data dianalisis menggunakan SPSS v.26 untuk windows. Hasil: suplementasi vitamin D3 dapat meningkatkan kadar 25(OH)D dengan hasil independent t test menyatakan adanya perbedaan yang signifikan (sig=0.000), menurunkan kadar NLR dengan hasil independent t test yang menyatakan adanya perbedan yang signifikan (sig=0.000). Hasil uji korelasi Pearson menunjukkan hubungan peningkatan kadar 25(OH)D dan membaiknya fungsi LFG, dimana didapatkan hubungan vi positif kuat (r=0,757), dan pada NLR dan membaiknya fungsi LFG didapatkan korelasi hubungan negatif sangat kuat (r=-0,779). Kesimpulan: Kami menyimpulkan bahwa efek suplementasi vitamin D3 dapat meningkatkan kadar plasma 25(OH)D dan menurunkan nilai NLR secara signifikan pada anak dengan PGK, dan didapatkan keterkaitan kuat antara peningkatan kadar 25(OH)D dan penurunan NLR dengan fungsi LFG
English Abstract
Background and objectives: Chronic kidney disease (CKD) is a serious health problem in children , with increasing morbidity and mortality. The relationship between vitamin D deficiency and inflammatory status is well known. Vitamin D has an important role in reducing the inflammatory burden. Several studies have shown that the neutrophil/lymphocyte ratio (NLR) is a potential marker for determining inflammation. So the purpose of this study was to determine the effect of vitamin D3 on plasma 25(OH)D levels, decreasing the value of Neutrophil to Lymphocyte Ratio (NLR), and glomerular filtration rate when compared with placebo in children with chronic kidney disease. Design, participants & measurements: This study was a pure experimental study with a randomized clinical trial (RCT) double blind, pre and post test control group with treatment in the form of oral vitamin D3 given for 12 weeks. The research group consisted of two groups, namely group 1 (K1) and group 2 (K2). The sample data normality test was performed using the Saphiro Wilk test. Differences in vitamin D levels, NLR values, and GFR were carried out by paired and independent t-tests. The relationship between vitamin D levels, NLR values and GFR was carried out by the Pearson. Data were analyzed using SPSS v.26 for windows. Results: Vitamin D3 supplementation increased 25(OH)D levels with independent t test indicating a significant difference (sig=0.000), decreasing NLR levels with independent t test indicating a significant difference (sig=0.000). And correlation Pearson showed a relationship between increased levels of 25(OH)D and improved GFR function, where there was a strong positive relationship (r=0.757), and at NLR and improving GFR function found a very strong negative correlation (r=-0,779). viii Conclusion: We concluded that the effect of vitamin D3 supplementation was. to increase plasma 25(OH)D levels and significantly decrease NLR values in children with CKD, and found a strong association between increased 25(OH)D levels and decreased NLR with GFR function
