Abstract
Triage menjadi hal yang penting untuk dilakukan di ruangan instalasi gawat darurat. Triage digunakan untuk menentukan prioritas pelaksanaan kegawatdaruratan sehingga tim kesehatan dapat melaksanakan penanganan untuk pasien yang sangat memerlukan penatalaksanaan dengan cepat serta tepat yang mampu meningkatkan peluang hidup pasien tersebut. Proses triage dimulai ketika pasien masuk di ruang IGD dimana perawat menilai berdasarkan pengkajian yang ditemukan baik secara subyektif maupun obyektif yang harus dilakukan dengan cepat dan tepat yang dilakukan oleh perawat. Namun, pelaksanaan triage ini membutuhkan pengetahuan, pengalaman, pendidikan dan lingkungan kerja yang mendukung dalam menentukan prioritas pasien. Penelitian ini bertujuan untuk mengidentifikasi faktor yang berhubungan pada perawat terhadap pengambilan keputusan triage di Instalasi Gawat Darurat Rumah Sakit Lombok Nusa Tenggara Barat. Penelitian ini merupakan penelitian observasional analitik dengan pendekatan cross sectional dengan jumlah responden sebanyak 135 orang. Hasil penelitian ini menunjukkan terdapat hubungan antara pengetahuan, pengalaman, pendidikan dan lingkungan kerja perawat terhadap pengambilan keputusan triage dengan masing-masing nilai p= 0.000. Variabel pengetahuan menjadi variabel yang paling berhubungan dengan pengambilan keputusan triage dengan nilai koefisien B 5.204. Pengetahuan, pengalaman kerja, pendidikan dan lingkungan kerja perawat mempengaruhi pengambilan keputusan triage. Untuk kedepannya, diharapkan adanya penentuan standar perawat yang bekerja diruangan triage sehingga pelaksanaan triage dapat berjalan dengan maksimal.
English Abstract
Triage is an important action to do in emergency unit, especially in categorizing emergency state level. It deals with management accuracy based on priority done by nurses. Triage process is done by emergency unit nurses in assessing patient conditions quickly and accurately. However, this process needs supportive knowledge, experience, education, and working environment to determine patients‟ priorities. Based on preliminary study done in October 31, 2018, there was problem in Lombok hospital in making triage process decision. Several of its nurses did not conduct triage accurately. They did mistake to determine priority of patients and to document appropriately. It negatively influenced emergency unit‟s service. This research aims to identify correlative factors of nurses in making triage decision in Emergency Unit of Lombok Nusa Tenggara Barat. This analytic observational research with cross sectional approach took 135 persons as respondents. The findings showed correlation among knowledge, experience, education, and working performance to triage decision making with each p score is 0.000. Knowledge becomes the most influential variable in triage decision making with coefficient score B 5.204. Knowledge, experience, education, and working environment influenced triage decision making. High nurses‟ knowledge about triage could improve accuracy in triage decision making in an emergency unit. The condition may help to categorize accurate triage based on condition suffered by patients. Thus, it may improve working performance and better outcomes. High experience could improve triage decision making. It became a learning for nurses to categorize triage accurately based on conditions of the patients to improve better working performance of the nurses as its outcome. Better education and competences of the nurses become main principle for them to do triage category assessment accurately based on patients‟ conditions, leading to better working performance and outcome. Better education of the nurses improved knowledge and skill of them in assessing triage quickly and accurately. Better working environment improved accuracy of the nurses to provide triage decision making in an emergency unit. It can be seen from good nursing staff and complete facility used by them to categorize triage accurately based on the conditions of patients so that working performance and outcome improved better. Better working environment improved efficiency and working performance of nurses maximally. Knowledge of the nurses becomes most contributive factor and becomes the most important in conducting triage. Nurses must have sufficient knowledge in handling patients especially those extremely severe or not and related to given treatment for each of them. It is expected to have standard requirement of emergency unit nurses in doing triage to run maximally.
