Abstract
Trauma kepala merupakan permasalahan yang terjadi di seluruh dunia, Trauma kepala memiliki hubungan erat dengan tingkat survival rate. Assessment klinis derajat keparahan trauma kepala berupa mekanisme trauma, lama waktu prehospital, Glasgow Coma Scale, tekanan darah sistolik dan usia memberikan gambaran secara kuantitatif dan memberikan gambaran survival rate. Penelitian ini bertujuan menganalisis hubungan antara hasil assessment klinis derajat keparahan dengan survival rate pasien trauma kepala di IGD RSUD dr. Soedono Madiun. Jenis penelitian ini merupakan observasional analitik cross sectional dan bersifat retrospektif. Sampel dalam penelitian ini adalah data rekam medis pasien trauma kepala sedang dan berat periode Januari 2017 hingga Desember 2018. Total sampel dalam penelitian ini adalah 180 rekam medis. Jenis kelamin dominan dalam penelitian ini adalah laki – laki sebanyak 129 pasien (71,7%) dan rerata usia 43,63 tahun. Hasil uji statistik chi square antara mekanisme trauma dan survival rate p 0,783 (p>0,05). Uji statistik rank spearman antara survival rate dengan lama waktu prehospital (p=0.000), GCS (p=0.000), tekanan darah sistolik (p=0.004), usia (p=0.006). Hasil regresi ordinal menunjukkan besarnya pengaruh assessment klinis terhadap survival rate 38,4%, sedangkan 61,6% survival rate dipengaruhi oleh faktor lain. Mekanisme trauma tidak memiliki hubungan dengan survival rate, sedangkan lama waktu prehospital, GCS, tekanan darah sistolik dan usia memiliki hubungan dengan survival rate. Assessment klinis dan tindakan pencegahan keparahan trauma kepala perlu dilakukan untuk meningkatkan survival rate
English Abstract
Head trauma has a close relationship with survival rate because it relates to pathophysiologic process. The survival rate of head trauma patients is affected by epidemiological factor, clinical factor and biochemical factor. The survival rate of head trauma patients is divided into immediate death, early death, and late death. Based on the result of a preliminary study, the total of patient visits to the Emergency Department was 24,354 patients with 924 of them included in the intermediate care category, consisting of cases of head trauma. 53.46% of patients experienced head trauma and the remaining 46.54% experienced non-head trauma. Clinical assessment of head trauma patients at the Emergency Department of the Regional Public Hospital Madiun uses trauma mechanism, length of prehospital time, Glasgow Coma Scale (GCS), systolic blood pressure, and age. It was also supported by a diagnostic examination such as CT Scan, which is not always available at other hospitals. Current identification of the assessment of the severity of head trauma is still a challenge to continue to develop, because trauma is a time sensitive condition, with regard to determining the needs of patients with limited resources. Based on the exposure of the research phenomenon, the aim of this study is to analyze the relationship between the result of clinical assessment of severity and the survival rate of head trauma patients in the Emergency Department of the Regional Public Hospital Madiun This is a quantitative retrospective research using the observational analytic method and cross-sectional approach. The sampling technique used purposive sampling, with the number of samples based on the inclusion and exclusion criteria, in which 180 medical records were retrieved. Data collection utilizes observation sheets. Of the 494 medical records reviewed, 180 medical records were used as the sample of this study. Data analysis was in accordance with the objectives formulated, and Chi Square test was used to analyze the relationship between the mechanism of trauma and survival rate. Rank Spearman was used to analyze the relationship of length of prehospital time, GCS, systolic blood pressure and age, with survival rate. Ordinal Regression Analysis was used to find out which variable affecting the survival rate of head trauma patients
