Abstract
Fraktur merupakan kondisi hilangnya kontinuitas tulang, yang dapat bersifat total maupun sebagian. Angka kejadian fraktur di seluruh dunia diperkirakan sekitar 4,5 juta, 740.000 kasus mengakibatkan kematian dan 1,75 juta menimbulkan kecacatan. Salah satu penatalaksanaan fraktur adalah dengan operasi Open Reduction Internal Fixation (ORIF). Pasien mengalami imobilisasi pasca operasi pemasangan ORIF dan diharuskan sudah dapat menggerakkan ujung-ujung jari tangan 6 jam pertama post operasi. Kondisi pasien studi kasus fraktur klavikula post operasi ORIF mengeluh tidak dapat menggerakkan tangannya setelah 6 jam post operasi. Oleh karena itu, pasien perlu diberikan intervensi dan edukasi mengenai mobilisasi dini. Hasil pengkajian pasien mengeluh nyeri, kekuatan otot tangan kanan 2, dan indeks barthel ketergantungan sedang. Diagnosis keperawatan pasien adalah nyeri akut, gangguan mobilitas fisik, gangguan integritas kulit/ jaringan, dan defisit perawatan diri. Intervensi yang diberikan adalah terapi relaksasi napas dalam dan kompres dingin untuk mengurangi nyeri akut; mobilisasi dini untuk meningkatkan mobilitas fisik; pemantauan tanda infeksi balutan luka; dan latihan akivitas perawatan diri. Latihan mobilisasi dini meliputi latihan gerakan ROM aktif, seperti fleksi dan ekstensi pergelangan tangan, abduksi dan adduksi pergelangan tangan, supinasi dan pronasi, mengepalkan jari tangan, abduksi dan adduksi jari-jari tangan. Selain itu, pasien juga diberikan ROM pasif sendi bahu hingga 90 derajat dan latihan meremas tisu. Pasien juga diberikan latihan miring kiri serta ROM aktif pada kedua kaki. Intervensi berlangsung selama tiga hari perawatan pasien. Evaluasi yang didapatkan adalah keluhan nyeri yang dirasakan pasien berkurang; rentang gerak dan kekuatan otot pasien meningkat; balutan post op bersih; pasien dapat toileting mandiri.
English Abstract
Fracture was a condition of bone continuity loss, it can be total or partial. The incidence of fractures in the worldwide was estimated at 4.5 million, 740,000 cases caused death and 1.75 million caused disability. One of the fracture management is Open Reduction Internal Fixation (ORIF). The patient was immobilized after post ORIF surgery and was required to be able to move the fingertips for the first 6 hours postoperatively. The condition of the case study patient ORIF postoperative clavicle fracture complained that she couldn’t move her hand after 6 hours postoperatively. Therefore, patients need to be given intervention and education about early mobilization. The assessment resulted patient complained of pain, right hand muscle strength scores 2, and barthel index score was moderate dependence. Nursing diagnosis of the patient is acute pain, impaired physical mobility, impaired skin/tissue integrity, and self-care deficit. The interventions given are deep breathing relaxation therapy and education about cold compresses; early mobilization to improve physical mobility; wound dressing monitoring; and self-care activities. Early mobilization exercises include exercises for active ROM movements, such as wrist flexion and extension, wrist abduction and adduction, supination and pronation, finger clenching, finger abduction and adduction. In addition, the patient was also given passive ROM of the shoulder joint up to 90 degrees and tissue squeezing exercises. The patient was also given left tilt body exercises and active ROM on both legs. The intervention lasted for three days of patient care. The evaluation obtained is that the pain felt by the patient is reduced; patient's range of motion increases; clean post op dressing; patient could do toileting independently.
