Abstract

Latar Belakang: Nefritis Lupus (NL) merupakan manifestasi serius pada Lupus Eritematosus Sistemik (LES) dan prediktor utama untuk prognosis buruk LES. Biomarker yang ada saat ini untuk mengevaluasi aktivitas dan kronisitas NL masih belum memuaskan, biopsi ginjal masih menjadi standar baku. Ureum dan kreatinin merupakan parameter yang banyak digunakan untuk mengetahui fungsi ginjal karena merupakan substansi endogen yang diproduksi relatif lebih stabil. Perubahan patologis pada membran basalis dan selektifitas protein akan menyebabkan suatu pola protein urine yang diekskresi. Identifikasi protein spesifik ini diharapkan membantu mengevaluasi keterlibatan ginjal yang berat. Tujuan: Mengetahui perbedaan gambaran protein spesifik urine pada NL dengan kadar ureum-kreatinin serum tinggi dan normal Metode: Penelitian deskriptif pada 20 orang kelompok sehat dan 20 orang kelompok NL. Pemeriksaan protein urine dilakukan dengan kromatografi kolom dan SDS-PAGE. Hasil: Terdapat perbedaan gambaran fragmen albumin dan jumlah peak pada kelompok NL dibandingkan kelompok sehat. Berat molekul protein spesifik urine pada kelompok sehat > 66kDa sedangkan kelompok NL ≤ 66kDa. NL dengan kadar ureum-kreatinin serum tinggi mempunyai protein urine tambahan yaitu protein dengan berat molekul rendah ( 21-30 kDa). Kesimpulan: Terdapat perbedaan gambaran, jumlah dan berat molekul protein spesifik urine pada kelompok sehat dan kelompok NL. NL dengan kadar ureum-kreatinin serum tinggi mempunyai protein urine tambahan yaitu protein dengan berat molekul rendah ( 21-30 kDa).

English Abstract

Background: Lupus Nephritis (LN) is the most serious manifestation in Systemic Lupus Erythematosus (SLE) and a major predictor for poor prognosis in SLE. Current biomarkers for evaluating activity and chronicity LN are considered to be unsatisfactory ad renal biopsy is still to be “gold sandart”. Ureum and creatinine are parameters that are widely used to determine kidney function because It is an endogenous substance produced relatively more stable. Pathological changes in glomerular basement membrane and selectivity of electrical charge are causing spesific patterns of urine proteins excretion. Identification of specific protein is expected to help evaluate severe kidney involvement. Objective: To examine the differences of urine specific protein patterns in NL with high and normal levels ureum-creatinine serum. Methods: A descriptive study in 20 normal group and 20 patients NL group. The patterns of urine specific proteins were determined using column chromatography and SDS-PAGE. Results: The pattern of urine specific proteins between normal group and case group are different in additional protein, total peaks and molecular weight. The weight of urine specific protein molecules in normal group> 66kDa while case group ≤ 66kDa. NL with high serum ureum-creatinine levels have additional urinary proteins that are low molecular weight proteins (21-30 kDa). Conclusions: There were differences in the patterns of the molecular weight of protein urine between normal group and case group. NL with high levels ureum-creatinine serum have additional urinary proteins that are low molecular weight proteins (21-30 kDa)
