Abstract
Preeklamsia merupakan komplikasi kehamilan yang menyebabkan lebih dari 25% kematian ibu di Indonesia. Pada kasus preeklamsia, peningkatan kadar Heat shock protein 90 (HSP90) dan Asymmetric dimethylarginine (ADMA) merupakan penanda suatu stres oksidatif dan inflamasi sistemik. Peningkatan HSP90 dan ADMA dapat merangsang terjadinya disfungsi endotel sehingga menyebabkan kerusakan vaskuler sistemik yang bermanifestasi menjadi sindrom preeklamsia seperti hipertensi, proteinuria dan edema. Extra virgin olive oil (EVOO) merupakan salah satu sumber antioksidan alami dari tanaman. EVOO memiliki profil asam lemak (FA) dengan rasio monounsaturated fatty acid (MUFA) atau asam lemak tak jenuh tunggal yang tinggi dan juga kandungan antioksidan yang kaya Penelitian dilakukan secara true experimental - post test only control group design. Menggunakan 20 ekor Rattus norvegicus yang dibagi menjadi 5 kelompok yaitu kelompok kontrol negatif (tikus bunting normal) , kontrol positif diinjeksi L-NAME (tikus model preeklamsia) , dosis 1 diinjeksi L-NAME+EVOO 0,5 mL/hari, dosis 2 diinjeksi L-NAME+EVOO 1 mL/hari dan dosis 3 diinjeksi L-NAME+ EVOO 2 mL/hari. L-NAME diinjeksi mulai hari ke 13 sampai 18 usia kehamilan, sedangkan EVOO diberikan sejak hari ke 1 sampai 18 usia kehamilan. Pengukuran tekanan darah dan protein urin dilakukan hari ke 12, 15 dan 19 usia kehamilan. Kemudian dilakukan terminasi untuk mengambil serum dan dilakukan pemeriksaan kadar HSP90 dan ADMA dengan ELISA. Penelitian ini menunjukan hasil adanya peningkatan kadar HSP90 dan ADMA pada kelompok tikus model preeklamsia dibandingkan kelompok tikus bunting normal. Hasil penelitian juga menunjukan bahwa kadar HSP90 dan ADMA menurun pada kelompok tikus yang diberikan EVOO dibandingkan dengan kelompok tikus model preeklamsia. Hasil penelitian menunjukan penurunan optimal kadar HSP90 pada kelompok EVOO dosis 1 mL/hari. Penurunan kadar ADMA optimal pada EVOO dosis 2 mL/hari. Hal ini terjadi karena flavonoid yang terdapat pada EVOO akan menghambat peroksidasi lipid pada tahap awal dengan perannya sebagai scavenger anion superoksida dan radikal hidroksil. Rantai reaksi radikal ini kemudian akan diputus dengan cara mendonasikan atom hidrogen kepada radikal peroksil sehingga terbentuklah radikal flavonoid yang akan bereaksi dengan radikal bebas untuk memutus rantai ROS. Extra virgin olive oil (EVOO) dapat menurunkan kadar HSP90 dan ADMA pada Rattus norvegicus model preeklamsia.
English Abstract
Preeclampsia is a complication in pregnancy that causes more than 25% of maternal deaths in Indonesia. In the case of preeclampsia, increased levels of Heat Shock Protein 90 (HSP90) and Asymmetric dimethylarginine (ADMA) are markers of oxidative stress and systemic inflammation. Increased HSP90 and ADMA can stimulate endothelial dysfunction resulting in systemic vascular damage which manifests into preeclampsia syndromes such as hypertension, proteinuria and oedema. Extra Virgin Olive Oil (EVOO) is a source of natural antioxidants from plants that have a fatty acid profile (FA) with a high ratio of monounsaturated fatty acids (MUFA) or monounsaturated fatty acids and also rich antioxidant content. The study was conducted using a true experimental - post-test only control group design. Twenty (20) Rattus norvegicus were used as models which were divided into 5 groups namely negative control group, positive control injected L-NAME, dose 1 injected L-NAME + EVOO 0.5 mL/day, dose 2 injected L-NAME + EVOO 1 mL/day and dose 3 injected L-NAME + EVOO 2 mL/day. L-NAME was injected from day 13 to 18 of gestational age, while EVOO was given from day 1 to 18 of gestational age. Measurements of blood pressure and urine protein were carried out on days 12, 15 and 19 of gestational age. Lastly, the termination was taken to take serum and HSP90 and ADMA levels were examined by ELISA. This study showed an increase in HSP90 and ADMA levels in the preeclampsia model of Rattus norvegicus group compared to the normally pregnant rats group. The results also showed that HSP90 and ADMA levels decreased in the rat group given EVOO compared to the positive control group. The optimal decrease in HSP90 levels is in the EVOO group at a dose of 1 mL/day. The optimal reduction in ADMA levels is in the EVOO group at a dose of 2 mL/day. It happened because the flavonoids contained in EVOO will inhibit lipid peroxidation at an early stage with its role as superoxide anion scavenger and hydroxyl radical. This chain of radical reactions will then be broken by donating hydrogen atoms to peroxyl radicals so that flavonoid radicals form which will react with free radicals to break the chain of ROS. Extra virgin olive oil (EVOO) can reduce HSP90 and ADMA levels in the preeclampsia model of Rattus norvegicus.
