Abstract

Latar Belakang : Tuberkulosis (TB) menjadi salah satu penyakit penyumbang kematian terbanyak di dunia. Vaksinasi BCG sebagai salah satu metode pencegahan infeksi TB memiliki manfaat dan lamanya perlindungan yang bervariasi sehingga dibutuhkan perkembangan vaksin baru. Vaksin baru yang dikembangkan menambahkan protein fusi rekombinan ESAT-6-CFP-10 yang tidak terdapat pada vaksin BCG. Kedua protein fusi rekombinan tersebut diharapkan dapat menginduksi respon imun yang optimal dalam melawan infeksi Mtb meliputi aktivasi sel T CD4+ dan sekresi sitokin IL-2 sebagai salah satu sitokin proinflamasi dan faktor protektif terhadap infeksi TB. Tujuan dari penelitian ini adalah untuk meneliti apakah protein fusi rekombinan ESAT-6/CFP-10 Mycobacterium tuberculosis (Mtb) dapat menstimulasi respons imun seluler, khususnya persentase sel T CD4+ dan IL-2 pada kultur PBMC. Metode : Penelitian eksperimental pada kultur PBMC dari 3 kelompok (pasien TB, kontak TB, dan sehat endemik). Setiap kelompok terdiri dari 8 sampel. Setiap kelompok diinduksi dengan tiga perlakuan yaitu tanpa antigen, PPD, dan protein rekombinan fusi ESAT-6/CFP-10 Mtb. Penelitian berlangsung selama 3 bulan. Persentase sel T CD4+ dan IL-2 diukur menggunakan flow cytometri. Hasil : Protein rekombinan fusi ESAT-6/CFP-10 meningkatkan persentase sel T CD4+ dan IL-2 pada pasien TB, kontak dan sehat endemik. Hasil persentase tertinggi dari sel T CD4+ diperoleh dari subyek sehat endemik (26,99%), yang tidak berbeda bermakna dibandingkan sampel kontak dan pasien TB (p=0,494). Hasil persentase tertinggi IL-2 ditemukan pada pasien TB (57,15%), yang berbeda bermakna dengan sehat endemik (p= 0,005). IL-2 pada kelompok kontak tidak memiliki perbedaan yang bermakna dengan kelompok sehat endemik (p=0,208) dan kelompok pasien TB (p=0,059). Kesimpulan : Protein fusi rekombinan ESAT-6-CFP-10 dapat meningkatkan persentase sel T CD4+ dan sitokin IL-2 pada ketiga kelompok perlakuan yang mendukung peran ESAT-6-CFP-10 sebagai kandidat vaksin TB yang mampu menginduksi respons protektif Th1

English Abstract

Background: Tuberculosis is one of the most contributor disease with high mortality in the world. BCG vaccination as a method of preventing TB infection has varying efficacy and durability so that the development of new vaccines is needed. The newly developed vaccine adds a recombinant ESAT-6-CFP-10 fusion protein that is not present in the BCG vaccine. Both recombinant fusion proteins are expected to induce an optimal immune response against Mtb infection including CD4 + T cell activation and cytokine IL-2 secretion as one of the proinflammatory cytokines and protective factors against TB infection. The aim of this study was to examine whether ESAT-6 / CFP-10 recombinant fusion protein Mycobacterium tuberculosis (Mtb) can stimulate cellular immune responses, specifically the percentage of CD4 + T cells and IL-2 cells in PBMC cultures. Methods: Experimental studies on PBMC cultures from 3 groups (TB patients, TB contacts, and endemic healthy controls). Each group consisted of 8 samples. Each group was induced with without antigen, PPD, and ESAT-6 / CFP-10 M.tb fusion recombinant protein. The study lasted for 3 months. The percentage of CD4 + and IL-2 T cells was measured using flow cytometry. Results: Recombinant ESAT-6 / CFP-10 fusion protein increased the percentage of CD4 + and IL-2 T cells in TB, contact and endemic healthy patients. The highest percentage results from CD4 + T cells were obtained from healthy endemic subjects (26.99 (8.89)), which were not significantly different from contact samples and TB patients (p = 0.494). The highest percentage of IL-2 was found in TB patients (57.15 (40.69)), which was significantly different from healthy endemic (p = 0.005). IL-2 in the contact group did not have a significant difference with the healthy endemic group (p = 0.208) and the TB patient group (p = 0.059). Conclusion: ESAT-6 / CFP-10 fusion protein is able to increase the percentage of CD4 + T cells and cytokines IL-2 in all research subject group which supports the role of ESAT-6-CFP-10 as a TB vaccine candidate that induces a Th1 protective response
