Abstract
Pencabutan gigi pada pasien dengan terapi antikoagulan berisiko mengalami perdarahan berlebih yang dapat mengganggu proses hemostasis dalam proses penyembuhan luka. Maka dari itu, pasien dengan terapi antikoagulan perlu diberi tindakan lokal yang mampu memberhentikan perdarahan dan mempercepat penyembuhan luka. Penelitian ini bertujuan untuk mengetahui pengaruh kitosan-gelfoam kulit kakao terhadap jumlah makrofag dalam mempercepat penyembuhan luka pasca ekstraksi gigi tikus Wistar dengan terapi antikoagulan. Metode penelitian ini menggunakan true experimental method dengan metode post-test only control group design dengan hewan uji berupa 24 tikus putih strain Wistar jantan yang diinjeksi heparin 0,09 ml/ 200 gr BB tikus. Hewan uji dibagi menjadi 8 kelompok menggunakan two time series, terdiri dari kelompok kontrol negatif, kelompok kontrol positif, dan kelompok perlakuan (kitosan-gelfoam ekstrak kulit kakao 1 ml dan 10 ml). Hewan uji didekapitasi pada hari ke-3 dan 7 dan dibuat preparat histologi menggunakan perwarnaan Hematoxylin Eosin. Pengamatan jumlah sel makrofag menggunakan mikroskop binokuler digital pada pembesaran 400x pada tiga lapang pandang. Hasil uji statistik menunjukkan terdapat perbedaan secara signifikan antar kelompok. Kelompok kontrol positif dan kelompok perlakuan menunjukkan adanya penurunan jumlah sel makrofag dari hari ke-3 ke hari ke-7, sedangkan kontrol negatif menunjukkan adanya peningkatan jumlah sel makrofag yang menandakan adanya inflamasi kronik. Kesimpulan penelitian ini adalah kitosan gelfoam ekstrak kulit kakao berpengaruh terhadap jumlah sel makrofag dalam mempercepat penyembuhan luka pada tikus Wistar jantan dalam terapi antikoagulan.
English Abstract
Tooth extraction in patients with anticoagulant therapy is at risk of excessive bleeding which can interfere with the hemostasis process in the wound healing process. Therefore, patients on anticoagulant therapy need to be given local measures that can stop bleeding and accelerate wound healing. This study aims to determine the effect of chitosan-gelfoam cacao pod husk on the number of macrophages in accelerating wound healing after tooth extraction of Wistar rats with anticoagulant therapy. This research method uses a true experimental method with a post-test-only control group design method. This study used 24 male Wistar white rats injected with heparin 0.09 ml/200 g body weight rats. The test animals were divided into 8 groups using two-time series, consisting of a negative control group, a positive control group, and two treatment groups (chitosan-gelfoam cacao pod husk extract 1 ml and 10 ml). The test animals were decapitated on days 3 and 7. Histological slides were made using Hematoxylin Eosin staining. The number of macrophage cells was observed using a digital binocular microscope at 400x magnification in three fields of view. The results of statistical tests showed that there were significant differences between groups. The positive control group and the treatment groups showed a decrease in the number of macrophages from day 3 to day 7, while the negative control group showed an increase in the number of macrophages which indicated chronic inflammation. This study concluded that chitosan-gelfoam cacao pod husk extract affected the number of macrophage cells in accelerating wound healing in male Wistar rats on anticoagulant therapy.
