Abstract
Latar Belakang: Obesitas adalah kondisi peradangan kronis tingkat rendah yang terkait dengan berbagai kondisi medis, seperti peningkatan sitokin inflamasi. Interleukin-6 (IL-6) memainkan peran sentral dalam regulasi inflamasi, hematopoiesis, dan respon imun. IL-6 juga dikaitkan dengan tingkat fungsi paru yang lebih rendah. Penelitian ini bertujuan untuk menganalisis pengaruh obesitas terhadap kadar IL-6, status faal paru, dan risiko OSA. Metode: analitik cross- sectional. Ada 127 peserta dengan 103 peserta obesitas dan 24 peserta non obesitas. Partisipan diwawancarai dengan kuesioner Epworth dan STOP-BANG, skor Epworth > 10 dan skor STOP-BANG 5 mengindikasikan OSA berisiko tinggi. Penilaian fungsi paru menggunakan spirometri, FVC <80% menunjukkan kelainan restriksi dan FEV1/FVC <70% menunjukkan gangguan obstruksi. Kadar serum IL-6 diukur menggunakan kit ELISA. Hasil: Sebanyak 59 partisipan obesitas (46,5%) mengalami gangguan restriksi. Terjadi peningkatan kadar IL-6 pada kelompok obesitas sebesar 221,30 (1.61 - 562.48) pg/dl (p=0,359). Subjek dengan obesitas tipe 3 (5%) memiliki risiko OSA lebih tinggi dengan skor STOP BANG 5 (P = 0,003). Terdapat perbedaan bermakna kadar IL-6 456.94 (3.62 – 542.38) pg/dl pada subjek dengan risiko OSA tinggi dibandingkan dengan risiko rendah dan sedang (P = 0,011). Kadar IL-6 tertinggi pada kelompok partisipan dengan gangguan paru obstruktif sebesar 540.65 (538.94 – 542.38) Kesimpulan: Obesitas menyebabkan peningkatan kadar IL-6, gangguan fungsi paru, dan peningkatan risiko OSA. English Abstract
Background: Obesity is a chronic low-grade inflammatory condition associated with various medical conditions, such as increase in inflammatory cytokines. Interleukin-6 (IL-6) plays a central role in the regulation of inflammation, hematopoiesis, and immune response. IL-6 is also associated with lower levels of pulmonary function. The study aims to analyze the effect of obesity on IL-6 levels, lung function status, and the risk of OSA. Methods: Cross-sectional analytic. There were 127 participants with 103 obese and 24 non obese participants. Participants were interviewed with the Epworth and STOP-BANG questionnaire, Epworth score >10 and STOP-BANG score ≥5 indicated high risk OSA. Pulmonary function assessment using spirometry, FVC <80% shows restriction abnormalities and FEV1/FVC < 70% indicated obstruction disorders. IL-6 serum levels were measured using the ELISA kit. Results: Total of 59 obese participants (46.5%) had restriction disorders. There was an increase in IL-6 levels in the obese group 221,30 (1.61 - 562.48) pg/dl (p=0.359). Those with type 3 obesity (5%) had a higher risk of OSA with STOP BANG score ≥ 5 (P = 0.003). There were significant difference in IL-6 levels 456.94 (3.62 – 542.38) pg/dl in participants with high risk of OSA compared to low and moderate risk (P = 0.011).
