Abstract
Latar Belakang: Tuberkulosis (TB) disebabkan oleh Mycobacterium tuberculosis masih menjadi penyebab utama kematian dan morbiditas di negara berkembang. M. tuberculosis telah menginfeksi hampir sepertiga dari populasi di seluruh dunia, hanya 10% dari orang yang terinfeksi M. tuberculosis akan berkembang menjadi TB aktif, sedangkan 90% individu yang terinfeksi akan menjadi TB laten tanpa gejala klinis, menunjukkan bahwa genetika host berperan penting dalam mengatur perkembangan penyakit. Studi asosiasi genetika sebelumnya telah mengidentifikasi berbagai lokus genetika host yang secara signifikan berkontribusi pada kerentanan TB. IL-6 adalah sitokin pro inflamasi yang disekresikan oleh monosit, sel endotel dan fibroblas, yang mampu merangsang limfosit B dan T, dan mengatur respon imun terhadap tuberkulosis. Dalam penelitian ini, kami meneliti hubungan polimorfisme gen IL-6 rs1800796 terhadap kerentanan penyakit TB paru pada pasien sensitif atau resisten obat. Metode : Desain penelitian yang digunakan adalah case control study. Penelitian dilakukan di Rumah Sakit Dr. Saiful Anwar Malang dari bulan Juli 2020 sampai Juni 2021. Subyek penelitian terdiri dari 105 subyek yang terbagi dalam 3 kelompok studi, 34 subyek kelompok kontrol sehat, 31 subyek TB SO, dan 40 subyek TB RO dilakukan pemeriksaan Polimorfisme Gen IL-6 rs1800796 dari sampel darah dengan Multiplex PCR. Hasil: Hubungan genotipe IL-6 rs1800769 (G/G, G/C, C/C) tidak signifikan (p > 0,05) dengan kerentanan terhadap TB Paru. Pada hasil pengujian hubungan antara Alel G dan Alel C tidak terdapat hubungan signifikan (p > 0.05) dengan kerentanan terhadap TB Paru. Kesimpulan: Tidak ditemukan adanya korelasi antara polimorfisme gen IL-6 rs1800796 terhadap kerentanan penyakit TB paru.
English Abstract
Introduction: Tuberculosis (TB) remains the main cause of mortality and morbidity worldwide, especially in developing countries. Indonesia is listed as the country with the second largest TB burden in the world according to the Global TB Report 2020. Among those infected with Mycobacterium tuberculosis, only 10% develop active tuberculosis, and most of infected individuals remain latently infected, implying that each stage of the host response to M. tuberculosis may be under genetic control. Growing number of reports have demonstrated that various host genetic factors are critical in susceptibility to tuberculosis. IL-6 is a pro-inflammatory cytokine secreted by monocytes, endothelial cells and fibroblasts, which is able to stimulate B and T lymphocytes, and regulate immune response to tuberculosis. In this study, we investigated the association of the IL-6 rs1800796 gene polymorphism on susceptibility to pulmonary tuberculosis in drug-sensitive or drug-resistant patients. Method: A case-control study involved 71 cases and 34 controls. A blood samples were collected from all study cases at the Saiful Anwar General Hospital Malang, during the period from August 2020 to February 2021.The blood samples were used for genotyping study with PCR-Multiplex technique to determine SNPs. Result: The genotype association of IL-6 rs1800796 (C/C, G/C, G/G) and the allele's frequency for patients and control of IL-6 rs1800796 were insignificant (P>0.05) with susceptibility to pulmonary tuberculosis. Conclusion: We found no evidence of association for IL-6 rs1800796 gene polymorphism with susceptibility to pulmonary tuberculosis.
