Abstract
Latar belakang : Pekerja industri marmer berisiko terpajan debu silika. Debu silika yang terhirup menyebabkan cedera, inflamasi, dan masuknya leukosit. Leukosit yang direkrut mengeluarkan sitokin profibrotik seperti TGF-β. Penelitian ini mengamati kadar TGF-β1 serum berdasarkan durasi paparan debu silika untuk mengetahui risiko penyakit terkait silika pada pekerja industri marmer. Metode: Studi observasional analitik potong lintang dilakukan pada 10 orang sehat tidak terpajan sebagai kelompok kontrol, sedangkan 40 pekerja terpajan debu silika industri marmer sebagai kelompok terpajan, kemudian dibagi ke dalam kategori yang berbeda berdasarkan durasi paparan: 1-5 tahun (12 ), 6-10 tahun (14), dan >10 tahun (14). Kadar TGF-β1 serum pada masing-masing kelompok ditentukan dengan ELISA. Uji Mann-Whitney dan Kruskal-Wallis digunakan untuk menyelidiki perbedaan antara kelompok dan kategori yang berbeda. Uji Spearman untuk mengetahui hubungan antara lama paparan debu silika dengan kadar TGF-β1 serum. Hasil: Median kadar TGF-β1 serum pada kelompok terpajan [337,63 pg/ml (26,48-12.542,16 pg/ml)] secara signifikan lebih tinggi daripada kelompok kontrol [104,84 pg/ml (47,84-253,13 pg/ml)] (p=0,029 ). Terdapat perbedaan bermakna kadar TGF-β1 serum antara kelompok terpajan >10 tahun [6.314,44 pg/ml (165,58-12.542,16 pg/ml)] dan kelompok kontrol (p=0,000), kelompok terpajan 1- 5 tahun [144,51pg/ ml (26,48-9,163,56 pg/ml)] (p=0,003), kelompok terpajan 6-10 tahun [205,35 pg/ml (27,32-11.707.02 pg/ml)] (p=0,003). Terdapat hubungan positif yang signifikan antara lama pajanan debu silika dengan kadar TGF-β1 serum (r=0,465 ; p=0,002). Kesimpulan: Pajanan debu silika dapat menyebabkan peningkatan kadar TGF- β1 serum pada pekerja industri marmer dan durasi pajanan memegang peranan yang penting.
English Abstract
Introduction : Marble industry workers are at risk of silica dust exposure. Inhaled silica dust cause an injury, inflammatory, and leukocytes entry. The recruited leukocytes secrete profibrotic cytokines such as TGF-β. This study observe serum TGF-β1 levels based on silica dust exposure duration to determine the risk of silica-related diseases in marble industry workers. Methode: A cross sectional analytic observational study was conducted on 10 non-exposed healthy people as control group, while 40 silica dust exposed workers of marble industry as exposed group, then divided into different categories based on exposure duration: 1-5 years (12), 6-10 years (14), and >10 years (14). Serum TGF-β1 levels in each group were determined with ELISA. Mann-Whitney and Kruskal-Wallis test are used to investigate the comparison between different groups and categories. Spearman-test to determine correlation between silica dust exposure duration and serum TGF-β1 levels. Results: The median serum TGF-β1 levels in exposed group [337.63 pg/ml (26.48-12,542.16 pg/ml)] were significantly higher than control group [104.84 pg/ml (47.84-253.13 pg/ml)] (p=0.029). There was a significant difference in serum TGF-β1 levels between >10 years exposed group [6,314.44 pg/ml (165.58-12,542.16 pg/ml)] and control group (p=0.000), 1-5 years exposed group [144.51pg/ml (26.48-9,163.56 pg/ml)] (p=0.003), 6-10 years exposed group [205.35 pg/ml (27.32-11,707.02 pg/ml)] (p=0.003). There was a significant positive correlation between silica dust exposure duration and serum TGF-β1 levels (r=0.465 ; p=0.002). Conclusion: Silica dust exposure can cause an increase in serum TGF-β1 levels in marble industry workers and exposure duration plays a important role.
