Abstract

Latar Belakang: Kegawatdaruratan medik merupakan risiko klinis yang dapat terjadi di berbagai tempat termasuk rumah sakit dan membutuhkan akses cepat terhadap obat. RS X mengadakan troli emergensi di unit rawat inap untuk mengurangi risiko ini. Penggunaan troli emergensi di unit rawat inap RS X sangat rendah dan berakibat kedaluarsa obat. Pengelolaan obat dalam troli emergensi membutuhkan manajemen logistik yang berfokus pada keselamatan pasien. Tujuan penelitian ini adalah melakukan analisis distribusi dan penyimpanan obat dalam troli emergensi unit rawat inap RS X sehingga dapat memberikan masukan kepada manajemen rumah sakit. Metode: Observasional deskriptif dengan pendekatan potong lintang pada 8 troli emergensi unit rawat inap RS X, rekaman penggunaan troli selama tahun 2018, kegiatan pengisian kembali, dan kegiatan supervisi. Hasil: Obat yang ada di dalam troli emergensi tidak selalu dapat memenuhi kebutuhan. Didapatkan 361 alasan penggunaan troli di unit rawat inap RS X yang dikelompokkan menjadi kondisi emergensi (70,64%), kondisi non emergensi (19,39%), dan tidak dapat dijelaskan (9,97%). Penataan obat dalam troli emergensi belum menjamin kecepatan pencarian dan penggunaan sesuai FEFO. Waktu pengisian kembali yang panjang berisiko pada keamanan obat dan keselamatan pasien akibat risiko kekosongan obat yang dibutuhkan. Perubahan yang dilakukan belum ditunjang penyesuaian regulasi. Kesimpulan: Distribusi dan penyimpanan obat dalam troli emergensi rawat inap belum dapat menjamin ketersediaan obat secara berkesinambungan dengan aman dan efisien. Perubahan regulasi terperinci terkait isi, penggunaan, dan pemeliharaan troli emergensi rawat inap RS X yang diikuti evaluasi paska implementasi sangat diperlukan. Penataan troli emergensi harus dapat mengakomodasi kebutuhan kecepatan akses maupun ketepatan distribusi dan penyimpanan.

English Abstract

Background: Medical emergencies are clinical risks that can occur in various places including hospitals and require quick access to drugs. X Hospital held an emergency trolley at each ward to reduce this risk. The use of emergency trolleys in X Hospital ward was very low and consequently expired drugs. Logistics management that focuses on patient safety is needed in order to manage emergency trolley's drugs. The purpose of this study was to analyze the distribution and storage of drugs in the emergency trolley of the X Hospital ward so as to provide input to the management. Methods: Observational descriptive cross sectional at 8 emergency trolley X Hospital ward, trolley usage records during 2018, drug replenishment activities, and supervision activities. Results: The drug in the emergency trolley was not always able to meet the needs. There were 361 reasons for the use of trolleys in the inpatient unit of X Hospital which were grouped into emergency conditions (70.64%), non-emergency conditions (19.39%), and unexplained (9.97%). Arrangement of emergency medicine in the cart does not guarantee the search speed and the use of appropriate FEFO. Long replenishment times pose a risk to drug safety and patient safety due to the risk of drug vacancies needed. Changes made have not been supported by regulatory adjustments. Conclusions: The distribution and storage of medicines in emergency hospitalization cart can not guarantee the continuous availability of the drug safely and efficiently. Detailed regulatory changes related to the content, use, and maintenance of emergency trolleys for X hospital followed by post-implementation evaluation are needed. Emergency trolley arrangement must be able to accommodate the needs of access speed and the accuracy of distribution and storage.
