Abstract

LATAR BELAKANG: Anemia hemolitik autoimun adalah kelainan langka yang ditandai oleh hemolisis, yaitu pemecahan sel darah merah yang terjadi oleh proses autoantibodi dan/atau komplemen, bersama dengan makrofag yang diaktifkan, limfosit T dan sitokin yang semuanya berkontribuasi pada prosesnya. Mekanisme yang terlibat dalam kegagalan toleransi pusat dan perifer dalam perkembangan AIHA sebagian besar masih belum diketahui. Limfopenia diketahui berperan dalam kejadian autoimun, demikian juga dengan sel T Helper 17 diketahui memiliki peran dalam patogenesisnya. METODE: Penelitian ini menggunakan desain observasional case control di laboratorium secara in vitro pada kultur peripheral blood mononuclear cell (PBMC) pada kelompok sampel pasien AIHA dan subjek normal. Pada penelitian ini akan dievaluasi persentase sel T CD4+ dan sel Th17 dengan flowsitometri. Uji normalitas dengan metode saphiro wilk. Perrbedaan antara kelompok dianalisis dengan independet students t-test/Mann Withney dengan tingkat kebermaknaan p < 0,05. Hasil dari analisa tersebut dilanjutkan dengan uji korelasi regresi linear. HASIL: Rerata persentase sel T CD4+ antara kelompok yang diuji dengan metode Mann Withney memiliki perbedaan signifikan (nilai p=0,000) dengan hasil Mean ± SD, T CD4+ KN 0,292 ± 0,024 KP 0,261 ± 0,006. Bila dibandingkan dengan KN terdapat penuruan sel T CD4+ pada kelompok AIHA.. Rerata persentase Th17 antara kelompok yang diuji dengan independent t-test tidak memiliki perbedaan bermakna dengan hasil Mean ± SD, Th17 KN 0,615 ± 0,053 KP 0,652 ± 0,042. KESIMPULAN: Persentase sel T CD4+ pada pasien AIHA lebih rendah dibandingkan subjek normal. Tidak ada perbedaan yang bermakna pada persentase sel Th17 pada kedua kelompok.

English Abstract

BACKGROUND: Autoimmune hemolytic anemia is a rare disorder characterized by hemolysis, the breakdown of red blood cells that occurs by autoantibody and/or complement processes, together with activated macrophages, T lymphocytes and cytokines all of which contribute to the process. The mechanisms involved in the failure of central and peripheral tolerance in the development of AIHA remain largely unknown. Lymphopenia is known to play a role in autoimmune events, as well as Th17 is known to have a role in its pathogenesis. METHOD: This study used a case-control observational design in the laboratory in vitro on peripheral blood mononuclear cell (PBMC) culture in the sample group of AIHA patients and normal subjects. In this study, the percentage of CD4+ T cells and Th17 will be evaluated by flow cytometry. Normality test using the saphiro wilk method. Differences between groups were analyzed by independent students t-test/Mann Withney with a significance level of p < 0.05. The results of the analysis are followed by a linear regression correlation test RESULTS: The mean percentage of CD4+ T cells between the groups tested using the Mann Withney method had a significant difference (p value=0.000) with the results Mean ± SD, T CD4+ KN 0.292 ± 0.024 KP 0.261 ± 0.006. When compared with KN, there was a decrease in CD4+ T cells in the AIHA group. The mean percentage of Th17 between the groups tested by independent t-test had no significant difference with the results Mean ± SD, Th17 KN 0.615 ± 0.053 KP 0.652 ± 0.042. CONCLUSION: The percentage of CD4+ T cells in AIHA patients was lower than in normal subjects. There was no significant difference in the percentage of Th17 in the two groups.
