Abstract

Non-alcoholic Steatohepatitis (NASH) merupakan subtipe dari penyakit Non-alcoholic Fatty Liver Disease (NAFLD). Secara histologi, NAFLD dikategorikan menjadi dua tipe, yaitu Non-alcoholic Fatty Liver (NAFL) yang menunjukkan perlemakan hati tanpa adanya bukti inflamasi berupa pembengkakan sel hepatosit, dan Non-alcoholic Steatohepatitis (NASH), yaitu adanya perlemakan hati disertai inflamasi berupa pembengkakan sel hepatosit, dengan atau tidak ada fibrosis. Diet tinggi lemak / High Fat Diet (HFD) merupakan diet dengan mengkonsumsi makanan berlemak dan berkolesterol tinggi yang dapat berisiko menyebabkan peningkatan kadar lipid dalam darah yang dikenal dengan istilah hyperlipidemia. NAFLD sering dikaitkan dengan kejadian sindroma metabolik, yaitu kumpulan gejala yang diakibatkan oleh peningkatan glukosa darah, tekanan darah, lemak abdominal, dan kolesterol. Tujuan penelitian ini adalah untuk mengetahui pengaruh pemberian HFD terhadap terjadinya NASH pada tikus Rattus norvegicus strain Wistar. Metode penelitian menggunakan true experimental study dengan menggunakan post-test only group design. Subjek penelitian menggunakan hewan coba tikus Rattus norvegicus strain Wistar yang diberi diet tinggi lemak (high fat diet) dan sebagai kontrol adalah hewan coba yang diberi diet normal. Uji hipotesis pada penelitian ini menggunakan analisis univariat, analisis bivariat dengan Uji Saphiro-Wilk, Levene’s Test, Uji independent T-Test, dan Uji Mann Whitney. Pada penelitian ini didapatkan bahwa pada pemberian HFD, gambaran histopatologi jaringan hepar menggunakan NASH CRN Scoring System: NAS dan Fibrosis score menunjukkan steatosis <5% sehingga tidak dapat didiagnosis sebagai NAFLD. Terjadi perbedaan kenaikan berat badan (BB) yang signifikan setelah diberikan perlakukan selama 12 minggu. Kadar gula darah tidak mengalami perbedaan yang signifikan dengan kelompok kontrol. Kadar fungsi hati, AST tidak memberikan perbedaan dengan kelompok kontrolnya, dan kadar ALT meningkat namun tidak signifikan secara statistik. Sedangkan kadar ALP menunjukkan perbedaan yang signifikan terhadap kelompok kontrol. Kadar bilirubin total, direk, indirek tidak meningkat secara signifikan. Kadar profil lipid (kolesterol total, TG, HDL, dan LDL) menunjukkan hasil yang tidak berbeda secara signifikan terhadap kelompok kontrol, meskipun didapatkan adanya peningkatan kadar TG dan LDL serta penurunan kadar HDL. Hasil penelitian ini menunjukkan pemberian HFD tidak berpengaruh terhadap terjadinya NASH. Terdapat hubungan bermakna antara variable kenaikan berat badan dengan pemberian HFD. Namun, tidak didapatkan hubungan bermakna antara variable gambaran histopatologi, kadar gula darah, fungsi hati, dan profil lipid dengan pemberian HFD.

English Abstract

Non-alcoholic Steatohepatitis (NASH) is a subtype of Non-alcoholic Fatty Liver Disease (NAFLD). Histologically, NAFLD is categorized into two types, namely Non-alcoholic Fatty Liver (NAFL) which shows fatty liver without evidence of inflammation in the form of swelling of hepatocytes, and Non-alcoholic Steatohepatitis (NASH), which is fatty liver accompanied by inflammation in the form of swelling of hepatocytes with or without fibrosis. High-fat diet (HFD) is a diet by consuming fatty foods and high cholesterol which can be at risk of causing increased levels of lipids in the blood, known as hyperlipidemia. NAFLD is often associated with the occurrence of metabolic syndrome, which is a collection of symptoms caused by an increase in blood glucose, blood pressure, abdominal fat, and cholesterol. The research method uses a true experimental study using a post-test only group design. The research subjects used Rattus norvegicus strain Wistar rats as experimental animals that were given a high fat diet (high fat diet) and as controls were experimental animals that were given a normal diet. Hypothesis testing in this study used univariate analysis, bivariate analysis with the Saphiro-Wilk Test, Levene's Test, Independent T-Test, and Mann Whitney Test. In this study, it was found that when HFD was given, the histopathological picture of liver tissue using the NASH CRN Scoring System: NAS and Fibrosis score showed steatosis <5% so it could not be diagnosed as NAFLD. There was a significant difference in weight gain (BB) after being given treatment for 12 weeks. Blood sugar levels did not experience a significant difference with the control group. Liver function levels, AST did not show any difference with the control group, and ALT levels increased but were not statistically significant. While the levels of ALP showed a significant difference to the control group. Total, direct, indirect bilirubin levels were not significantly increased. Lipid profile levels (total cholesterol, TG, HDL, and LDL) showed results that were not significantly different from the control group, although there was an increase in TG and LDL levels and a decrease in HDL levels. The results of this study indicate that the administration of HFD has no effect on the occurrence of NASH. There is a significant relationship between the variable weight gain and the administration of HFD. However, there was no significant relationship between histopathological picture variables, blood sugar levels, liver function, and lipid profile with HFD administration.
