Abstract
Sasaran keselamatan pasien sebagaimana yang ditetapkan dalam Peraturan menteri kesehatan Nomor 11/ MENKES/ 2017 tentang “sasaran keselamatan pasien” menuntut manajemen operasional Rumah Sakit berkomitmen menjalankan semua prosedur pelayanan kesehatan dengan benar, meningkatkan cara berkomunikasi yang efektif, meningkatkan keamanan obat – obatan yang harus diwaspadai, memastikan lokasi pembedahan yang benar dan prosedur yang benar serta pembedahan pasien yang benar, mengurangi risiko infeksi akibat perawatan kesehatan dan mengurangi risiko cidera pada pasien akibat terjatuh. Kondisi di RSUD Datoe Binangkang Bolaang Mongondow yang dapat diamati pada tahun 2017 – 2018, kesalahan – kesalahan penanganan dan kejadian buruk yang sebenarnya dapat dihindari tergolong cukup tinggi antara lain. (1) Pasien mengalami infeksi pada prosedur pemasangan infus (plebitis) (2) Kejadian pasien jatuh (3) Kasus kesalahan pada pemberian obat (4) Kasus kesalahan pada tindakan operasi Dari hasil pengamatan, kesalahan/ kejadian lebih disebabkan oleh beberapa faktor, yaitu : (1) Faktor komunikasi yang belum efektif (2) Faktor kualitas sumber daya manusia yang masih kurang (3) Faktor tidak menjalankan prosedur tindakan yang benar (4) Faktor sarana keselamatan pasien yang belum memadai (5) Faktor fungsi pengawasan yang belum optimal (6) Faktor biaya operasional Rumah Sakit dari alokasi anggaran yang berasal dari pemerintah daerah belum dapat memenuhi kebutuhan pembiayaan penerapan standar – standar keselamatan Kajian penelitian kualitatif dengan pendekatan studi kasus, telah mendorong perhatian peneliti untuk melakukan penelitian Analisis Penerapan Keselamatan Pasien (Patient Safety) di RSUD “Datoe Binangkang”. Tujuannya adalah : (1) Untuk menganalisis indikator variabel profil sistem manajemen operasional Rumah Sakit terhadap Program manajemen mutu sasaran keselamatan pasien di RSUD “Datoe Binangkang” (2) Untuk menganalisis indikator variabel kinerja sumber daya manusia (kinerja karyawan) yang diduga berperan terhadap timbulnya masalah pada manajemen patient safety and quality di RSUD “Datoe Binangkang” Analisis hasil studi menggunakan cara : perjodohan profil, deret waktu, model logik, serta eksplanasi akan menunjukkan berbagai hasil positif terhadap manajemen operasional di Rumah Sakit meliputi aspek input (masukan), proses, output (keluaran) dan outcome yang akan memberi dampak terhadap dikembangkannya model manajemen mutu dan keselamatan pelayanan xi kesehatan yang berkualitas saat ini, karena selama ini penelitian mutu dan keselamatan pasien di Rumah Sakit masih jarang dilakukan. Analisa SWOT : Strength (kekuatan), Weakness (Kelemahan), Opportunity (kesempatan), Threat (ancaman) terhadap model Safety and Quality perlu dilakukan untuk melengkapi kajian hasil analisis studi kasus ini juga telah memberi wawasan yang maju terhadap pengembangan penerapan keselamatan pasien yang bermutu di Rumah Sakit, sebagai upaya melakukan evaluasi keberhasilan pelayanan kesehatan. Kesimpulan penelitian akan menghasilkan saran yang baik bagi peneliti dan bagi stakeholder sebagai pengambil kebijakan. (1) Peneliti sebagai human rasional dibekali oleh Sang Pencipta Hasrat ingin tahu. Dorongan ingin tahu inilah peneliti berusaha mencari Ilmu Pengetahuan yang benar mengenai hal – hal yang selama ini menjadi pertanyaan dan dipertanyakan yaitu tentang berpikir dan berbuat sesuatu yang benar. (2) Hasil penelitian ini akan dapat menjawab Asumsi Peneliti, bahwa dengan menganalisis Penerapan Keselamatan Pasien (Patient Safety) di RSUD Datoe Binangkang, dapat memberi solusi bagaimana mengatasi masalah yang diduga berperan terhadap timbulnya masalah keselamatan yang seharusnya dapat dicegah.
English Abstract
The regulation of the minister of health II/MENKES/2017 about “Patient Safety Target” states that hospital operational management should committed to run all of the health service procedures correctly including the right patient identification process, effective communication, the awareness of drugs used, certainty clear about the surgery location with the right procedures, also reduces the risks of infection due to the health treatment and reduces the risks of injury of the patient. As determined in the observation, the condition of RSUD Datoe Binangkang Bolaang Mongondow in 2017 to 2018 shows that there are some mishandling which can be avoided are quite high, such as : (1) Patient infection caused by wrong infusion procedure (Phlebitis) (2) Patient injury (3) The errors in drugs administration (4) The case of surgery error Based on the observation, those mistakes are most likely caused by some factors, which are: (1) Non-effective communication (2) The lack of human resources quality (3) The wrong execution of patient procedures (4) Inadequate patient safety facilities (5) The role of supervisor that not optimal (6) Hospital operational cost from the government funding budget still cannot meet the financial needs of safety standardization. This research is using qualitative approach to analyze patient safety application in RSUD Datoe Binangkang. The objectives of the study are: (1) To analyze variable indicator profile in the hospital operational management system to the management programme of the patient safety quality in RSUD Datoe Binangkang (2) To analyze the variable indicator human resources performance which affects the problem of safety and quality management in RSUD Datoe Binangkang The result of this analysis uses profile matching, timetable, logic model, also explanation that will shows some positive results of hospital operational management such as input aspects, process. output aspects and outcome that will give some affects to the quality of the management and health service. In fact, the analysis of quality and patient safety in the hospital is still rarely done on this day. The analysis of S. W. O. T (Strength, Weakness, Opportunity, Threat) to the model of safet and quality should be done to complete the result of this study and gives an advance insight to the development of the application of patient safety in a hospital, as an effort to do ti valuation of health service. xiii The conclusions of this research are give the positive suggestions to the researcher and the stakeholder as a policy maker : (1) The researcher as a rational humal is given by God a curiosity. Along with this curiosity, the researcher gives an effort to seek the truth of science about the unknown stuffs questioned by human which are thinking and do something correctly (2) The result of the study will answer the researcher assumption that analyzing the application of patient safety in RSUD Datoe Binangkang can give solutions on how to resolve problems that affect the safety error which can be prevented.
