Abstract
Diabetes melitus (DM) adalah salah satu penyakit dengan prevalensi tertinggi di dunia. Di indonesia sendiri, prevalensi DM mencapai angka 2%. Diabetes melitus berpotensi mengalami komplikasi, salah satunya yakni nefropati diabetik (ND). Marker yang dapat digunakan sebagai penanda kerusakan ginjal pada ND adalah nefrin. Beberapa studi terbaru telah membuktikan bahwa nefrin lebih baik dalam mendeteksi kerusakan ginjal lebih awal daripada albuminuria. Penatalaksanaan terapi DM sangat penting untuk mencegah dan menghambat progresivitas terjadinya komplikasi. Salah satu tanaman yang sudah dikenal memiliki efek antidiabetes yakni Tithonia diversifolia. Dosis ekstrak daun Thitonia diversifolia yang digunakan pada penelitian ini adalah 50 mg/KgBB, 100 mg/KgBB dan 150 mg/KgBB. Enam kelompok. 36 tikus wistar dibagi dalam 6 kelompok yakni kelompok normal (non-diabetes), kelompok positif (diabetes tanpa diberi terapi), 3 kelompok diberi terapi Tithonia, dan 1 kelompok diberi terapi glikuidon. Urin tikus DM yang telah diberi perlakuan selama 42 hari akan dikumpulkan dan diukur konsentrasi nefrin urin menggunakan metode ELISA. Dari hasil pengukuran didapatkan kadar nefrin urin pada kelompok kontrol positif (KP) meningkat secara signifikan jika dibandingkan dengan kelompok kontrol negatif (KN) (p<0,05). Pada semua kelompok yang diberi terapi ekstrak T. diversifolia mengalami penurunan konsentrasi nefrin secara signifikan (p<0,05). Penelitian ini mengungkapkan bahwa pemberian ekstrak T. diversifolia dapat menurunkan kadar nefrin pada urin tikus DM. Namun, diperlukan adanya penelitian lanjutan dengan parameter lainnya seperti kreatinin urin untuk menentukan derajat keparahan nefropati diabetik.
English Abstract
Diabetes mellitus (DM) is one of the diseases with the highest prevalence in the world. In Indonesia, the prevalence of DM reaches 2%. Diabetes mellitus has the potential to experience complications, one of which is diabetic nephropathy (ND). The marker that can be used as a marker of kidney damage in ND is nephrin. Several recent studies have shown that nephrin is better at detecting kidney damage earlier than albuminuria. One of the plants that are known to have anti-diabetic effects is Tithonia diversifolia. The doses of T. diversifolia leaf extract used in this study were 50 mg/kg BW, 100 mg/kg BW, and 150 mg/kg BW. The 36 Wistar rats were divided into 6 groups, namely the nondiabetic group, the positive group, 3 groups receiving Tithonia therapy, and gliquidone group therapy. The urine of DM rats that have been treated for 42 days will be collected and the urine nephrin concentration measured using the ELISA method. From the measurement results, it was found that urine nephrin levels in the positive control group increased significantly when compared to the non-diabetic group (p<0.05). In all groups treated with T. diversifolia extract, the nephrin concentration decreased significantly (p<0.05). This study revealed that the administration of T. diversifolia extract could reduce nephrin levels in the urine of DM rats. However, further research is needed with other parameters such as urine creatinine to determine the severity of diabetic nephropathy.
