Abstract

Diabetes melitus merupakan penyakit metabolik yang ditandai dengan adanya hiperglikemia akibat gangguan sekresi insulin, kerja insulin, atau keduanya. Salah satu komplikasi diabetes adalah nefropati diabetik. Pada nefropati diabetik, terjadi penurunan ekspresi protein utama celah diafragma podosit yang berperan penting dalam mempertahankan filtrasi glomerulus yaitu nefrin. Beberapa penelitian melaporkan bahwa daun insulin ( Tithonia diversifolia) memiliki efek antidiabetes. Penelitian ini bertujuan untuk mengetahui efektivitas ekstrak daun insulin terhadap ekspresi nefrin pada glomerulus ginjal diabetes. Penelitian eksperimental murni menggunakan randomized post-test only controlled group design dilakukan terhadap hewan coba tikus Wistar jantan yang diinduksi STZ 45 mg/kg untuk diabetes. Sampel diambil dengan teknik siml pie random sampling dan dibagi ke dalam 6 kelompok, yaitu kelompok kontrol negatif (normal), kontrol positif (diabetes tanpa terapi), P1 (ekstrak daun insulin 50 mg/kg), P2 (ekstrak daun insulin 100 mg/kg), P3 (ekstrak daun insulin 150 mg/kg), dan P4 (glikuidon 10 mg/kg). Penelitian dilakukan selama 42 hari dan ekspresi nefrin dinilai menggunakan metode imunohistokimia. Hasil pada penelitian ini menunjukkan bahwa penurunan ekspresi nefrin pada kelompok diabetes tidak berbeda bermakna dengan kelompok kontrol negatif (ANOVA, p=0.179). Namun, terjadi peningkatan ekspresi nefrin yang signifikan pada kelompok P3 dan P4 dibandingkan kontrol positif (P3, p=0.041; P4, p=0.029). Maka dari itu, dapat disimpulkan bahwa ekstrak daun insulin mampu meningkatkan ekspresi nefrin pada ginjal diabetes pada dosis 150 mg/kg. Berdasarkan penelitian ini, disarankan untuk meneliti dosis maksimal ekstrak daun insulin yang dapat digunakan untuk pengobatan diabetes melitus, terutama pengaruhnya terhadap perbaikan kondisi ginjal pada diabetes melitus.

English Abstract

Diabetes mellitus is a metabolic disease characterized by hyperglycemia due to impaired insulin secretion, insulin action, or both. One of the complications of diabetes is diabetic nephropathy. In diabetic nephropathy, there is a decrease in the expression of the main protein in the podocyte diaphragm slit that plays an important role in maintaining glomerular filtration, namely nephrin. Several studies have reported that insulin leaf ( Tithonia diversifolia) has antidiabetic effects. This research was aimed to determine the effectiveness of insulin leaf extract on nephrin expression in the diabetic kidney glomerulus. A true experimental study using a randomized post-test only control group design was conducted on male Wistar rats induced by STZ 45 mg/kg for diabetes. The samples were selected by simple random sampling technique and divided into six groups, that were negative control (normal), positive control (diabetes without therapy), P1 (insulin leaf extract 50 mg/kg), P2 (insulin leaf extract 100 mg/kg), P3 (insulin leaf extract 150 mg/kg), and P4 (gliquidone 10 mg/kg). The study was conducted for 42 days and nephrin expression was examined using immunohistochemical methods. The result of this research showed that the decrease in nephrin expression in the diabetes group was not significantly different from that in the negative control group (ANOVA, p=0.179). However, there was a significant increase in nephrin expression in the P3 and P4 groups compared to the positive control (P3, p=0.041; P4, p=0.029). Thus, it was concluded that insulin leaf extract can increase nephrin expression in diabetic kidneys at a dose of 150 mg/kg. Based on this research, it was suggested to examine the maximum dose of insulin leaf extract that can be used to treat diabetes mellitus, especially its effect on improving kidney conditions in diabetes mellitus.
