Abstract

Erupsi obat didefinisikan sebagai suatu respon abnormal tubuh yang berbahaya terhadap bahan obat atau metabolitnya dalam rentang dosis normal. Manifestasi terbanyak kasus erupsi obat terjadi pada kulit yang dapat dikategorikan menjadi dua, yaitu erupsi obat berat dan erupsi obat tidak berat. Erupsi obat berat merupakan erupsi obat yang beresiko tinggi menimbulkan morbiditas persisten hingga mortalitas, seperti Steven Johnson Syndrome (SJS), Toxic Epidermal Necrolysis (TEN), Drug Reaction with Eosinophilia and Systemic Symptomps (DRESS) dan Acute Generalized Exanthematous Pustulosis (AGEP). Adapun erupsi obat tidak berat mencakup manifestasi yang meliputi urtikaria, angioedema kulit, eksantema makulopapular, dan Fixed Drug Eruption (FDE). Tiga golongan obat utama yang diduga memicu erupsi obat antara lain obat anti inflamasi non-steroid (OAINS), antibiotik, dan antikonvulsan. Tujuan dari penelitian ini adalah untuk mengetahui hubungan antara riwayat penggunaan obat golongan OAINS, antibiotik dan antikonvulsan dengan manifestasi klinis erupsi obat berat dan tidak berat di RSUD Dr. Saiful Anwar Malang. Penelitian memiliki desain studi cross sectional dan menggunakan data rekam medis Poli Kulit dan Kelamin RSUD Dr. Saiful Anwar Malang periode 2018-2020. Data dianalisa menggunakan software SPSS versi 23 dengan metode uji Chi-Square dengan taraf nilai-p 0,05. Hasil uji statistik pada 70 pasien menunjukkan tidak terdapatnya hubungan yang signifikan antara riwayat penggunaan masing-masing obat golongan OAINS (p = 0,617; OR = 0,603 (0,176-2,069)), antibiotik (p = 0,351; OR = 0,565 (0,217-1,471)), dan antikonvulsan (p = 0,071; OR = 3,382 (1,032-10,980)) dalam menimbulkan manifestasi klinis erupsi obat berat.

English Abstract

Drug eruption is defined as an abnormal response of the body to a drug substance or its metabolites within the normal range of doses. The manifestations of the most cases of eruptions on the skin can be classified into two categories, namely severe drug eruptions and non-severe drug eruptions. Severe drug eruptions are perilous drug eruptions that can pose persistent morbidity and mortality, such as Steven-Johnson Syndrome (SJS), Toxic Epidermal Necrolysis (TEN), Drug Reaction with Eosinophilia and Systemic Symptomps (DRESS), and Acute Generalized Exanthematous Pustulosis (AGEP). The non-severe drug eruption, however, covers the cutaneous manifestation ranging from urticaria, skin angioedema, maculopapular exanthema, and Fixed Drug Eruption (FDE). The three main drug classes that are suspected as culprit in inducing drug eruptions include non-steroidal anti-inflammatory drugs (NSAIDs), antibiotics, and anticonvulsants. The purpose of this study was to determine the relation between the past medication history of NSAIDs, antibiotics and anticonvulsants with clinical manifestations of severe and non-severe drug eruptions in Dr. Saiful Anwar Malang. The research has a cross sectional study design and uses medical records from the Dermatology and Venereology Clinic at Dr. Saiful Anwar Malang period 2018-2020. Data analysis used SPSS version 23 software with Chi-Square test method with p-value level of 0.05. The results of statistical tests on 70 patients showed that there was no significant relation between the past medication history of respective NSAID (p = 0.617; OR = 0.603 (0.176-2.069), antibiotics (p = 0.351; OR = 0.565 (0.217-1.471)), and anticonvulsants class drug (p = 0.071; OR = 3.382 (1.032-10.980)) with the severe clinical manifestations of drug eruption patients.
