Abstract

Sepsis merupakan penyebab kematian terbanyak di seluruh dunia. Insiden sepsis di seluruh dunia meningkat dari tahun ke tahun. Pada sepsis terjadi inflamasi sistemik yang menyebabkan disfungsi organ sehingga diperlukan biomarker untuk menentukan diagnosa dan prognosa sepsis. Laktat Dehidrogenase (LDH) merupakan salah satu biomarker kerusakan organ yang berhubungan dengan outcome sepsis. Tujuan penelitian ini adalah mengetahui hubungan antara LDH dengan outcome pasien sepsis di RS Saiful Anwar Malang. Desain penelitian adalah observasional analitik dengan pendekatan kohort prospektif. Metode sampling yang digunakan adalah consecutive sampling. LDH diukur menggunakan Roche Cobas 8000. Outcome yang dilihat adalah mortalitas dan lama rawat yang diperoleh dari rekam medis. Hasil penelitian menunjukkan tidak terdapat perbedaan kadar LDH berdasarkan mortalitas pasien sepsis (p = 0,414), terdapat hubungan negatif antara kadar LDH dengan lama rawat pasien sepsis (r = -0,576,p = 0,031), tidak terdapat hubungan antara kadar LDH dengan mortalitas pasien sepsis (r = 0,14,p = 0,414), serta didapatkan cut-off LDH terhadap mortalitas sebesar 728U/I dan nilai Relative Risk adalah 1,28. Pemeriksaan LDH dilakukan hanya 1 kali dan manajemen sepsis diduga mempengaruhi hasil penelitian ini. Kesimpulan penelitian adalah kadar LDH memiliki hubungan sedang dengan lama rawat pasien sepsis, namun tidak memiliki hubungan dengan mortalitas pasien sepsis.

English Abstract

Sepsis is the leading cause of death worldwide. The incidence of sepsis worldwide has increased from year to year. In sepsis there is a systemic inflammation that causes organ dysfunction, so a biomarker is needed to determine the diagnosis and prognosis of sepsis. Lactate dehydrogenase (LDH) is a biomarker of organ damage related to sepsis outcome. The purpose of this study was to determine the relationship between LDH and sepsis patient outcomes at Saiful Anwar Hospital Malang. The study design was observational analytic with a prospective cohort approach. The sampling method used was consecutive sampling. LDH was measured using Roche Cobas 8000. The outcome seen was mortality and length of stay obtained from medical records. The results showed there were no differences in LDH levels based on the mortality of sepsis patients (p = 0.414), there was a negative relationship between LDH levels with the length of stay in sepsis patients (r = -0.576, p = 0.031), there was no relationship between LDH levels with the mortality of sepsis patients (r = 0.14, p = 0.414), and the LDH cut-off for mortality was 728U / I and the Relative Risk value was 1.28. LDH examination was done only 1 time and management of sepsis was thought to influence the results of this study. The conclusion of the study was that LDH levels had a moderate relationship with length of stay in sepsis patients, but did not have a relationship with mortality in sepsis patients.
