Abstract
Tuberkulosis (TBC) masih menjadi masalah kesehatan di dunia. Menurut Global Tuberculosis Report 2018, Indonesia berada pada peringkat ke-3 (8%) negara dengan beban TBC yang tinggi (high burden countries). Proporsi kasus TBC anak di Indonesia meningkat dari 7,16 % pada tahun 2014 menjadi 10,08 % pada tahun 2017. Pengobatan fase intensif pada TBC anak diberikan setiap hari selama 2 bulan pertama menggunakan regimen terapi Isoniazid, Rifampisin, dan Pirazinamid. Pengobatan TBC dapat menimbulkan gangguan hematologi berupa anemia karena proses inflamasi TBC, malnutrisi maupun efek samping obat anti-TBC. Tujuan penelitian ini adalah mengetahui perbedaan kadar Hemoglobin, Indeks Eritrosit dan Red Cell Distribution Width (RDW) sebelum dan sesudah terapi intensif pada TBC anak. Penelitian ini menggunakan desain observasional analitik dengan pendekatan kohort. Didapatkan 30 pasien anak dengan TBC paru baru dan diikuti selama 2 bulan. Hasil penelitian menunjukkan subjek penelitian terbanyak adalah laki-laki (n = 18), usia kurang dari 5 tahun (n = 15) dan status gizi baik (n = 16). Tidak didapatkan perbedaan kadar Hemoglobin, Indeks Eritrosit dan RDW sebelum dan sesudah terapi intensif (nilai p berturut-turut 0,750; 0,646; 0,239; 0,054; dan 0,271, α < 0,05). Hal tersebut menunjukkan bahwa pemberian OAT standar pada anak tidak menyebabkan efek samping.
English Abstract
Tuberculosis (TB) is still a health problem in the world. According to the 2018 Global Tuberculosis Report, Indonesia is ranked 3rd (8%) of countries with high TB burden (high burden countries). The proportion of cases of TB in children in Indonesia increased from 7.16% in 2014 to 10.08% in 2017. Intensive phase treatment in pediatric TB is given every day for the first 2 months using isoniazid, rifampicin, and pirazinamid treatment regimens. TB treatment can cause hematological disorders in the form of anemia due to the inflammatory process of tuberculosis, malnutrition and the side effects of anti-TB drugs. The purpose of this study was to determine differences in hemoglobin levels, RBC Indices and Red Cell Distribution Width (RDW) before and after intensive therapy in pediatric tuberculosis. This study uses an observational analytic design with a cohort approach. There were 30 pediatric patients with new pulmonary tuberculosis and were followed for 2 months. The results showed the most research subjects were male (n = 18), age less than 5 years (n = 15) and good nutritional status (n = 16). There were no differences in hemoglobin levels, Erythrocyte Index and RDW before and after intensive therapy (p values respectively 0.750; 0.646; 0.239; 0.054; and 0.271, α < 0.05). This shows that giving standard OAT to children does not cause side effects.
