Abstract
Seksio sesarea (SC) adalah proses melahirkan untuk mengeluarkan bayi melalui operasi pada dinding perut dan rahim. Setiap operasi akan memerlukan anestesi umum atau Regional/ Sub Arachnoid Block (SAB). Teknik SAB dapat mempengaruhi saraf sensorik dan motorik yang mengakibatkan perubahan status hemodinamik pasien. Untuk mengetahui efek SAB setalah operasi, salah satu indikatornya adalah bromage score (BS) yaitu untuk mengetahui pergerakan motorik ektrimitas bawah pasien. Penelitian ini menggunakan desain penelitian analitik observasional dengan pendekatan cross sectional, ditulis dilembar observasi dengan metode total sampling. Tujuan dari penelitian ini adalah untuk mengetahui hubungan antara status hemodinamik pasien SC dengan anestesi SAB sampai operasi selesai dengan pencapaian nilai BS ≥2 di ruang Recovery Room (RR) di RSUD dr. Saiful Anwar Malang. Jumlah responden berjumlah 36 orang sesuai dengan kriteria total sampling. Semua responden diamati dan dianalisis untuk status hemodinamik selama pemberian SAB sampai operasi selesai dalam bentuk tekanan darah dan denyut nadi dan diamati dalam RR sampai pencapaian nilai BS ≥2. Dari hasil analis menggunakan uji korelasi kontingensi, ditemukan bahwa dalam penelitian ini ada korelasi yang signifikan antara nilai p 0,00 dan koefisien korelasi 0,524 (sedang), dari 61,1% responden dengan status hemodinamik tidak stabil didapatkan 52,8% telah mencapai nilai BS ≥2 lebih dari 60 menit. Status hemodinamik pasien yang tidak stabil dalam 15 menit kedua setelah pemberian SAB dapat mempengaruhi durasi pencapaian bromage score di recovery room.
English Abstract
Caesarean section (SC) is the process of giving birth to expel a baby through surgery on the abdominal wall and uterus. Each operation will require general anesthesia or Regional / Sub Arachnoid Block (SAB). The SAB technique can affect the sensory and motor nerves resulting in changes in the patient's hemodynamic status. To determine the effect of SAB after surgery, one of the indicators is the bromage score (BS), which is to determine the motor movements of the patient's lower extremities. This study uses an observational analytic research design with a cross sectional approach, written on an observation sheet with a total sampling method. The purpose of this study was to determine the relationship between the hemodynamic status of SC patients with SAB anesthesia until the operation was completed with the achievement of the BS value ≥2 in the Recovery Room (RR) room at RSUD dr. Saiful Anwar Malang. The number of respondents amounted to 36 people according to the total sampling criteria. All respondents were observed and analyzed for hemodynamic status during the administration of SAB until the operation was completed in the form of blood pressure and pulse and observed in RR until the achievement of BS values BS2. From the results of analysts using the contingency correlation test, it was found that in this study there was a significant correlation between the p value of 0.00 and the correlation coefficient of 0.524 (moderate), from 61.1% of respondents with an unstable hemodynamic status it was found that 52.8% had reached the value BS ≥2 more than 60 minutes. The patient's unstable hemodynamic status within the second 15 minutes after the administration of SAB can affect the duration of achieving a bromage score in the recovery room.
