Abstract
Lupus Eritematosus Sistemik (LES) merupakan penyakit autoimun kronis dengan jumlah kematian yang meningkat tiap tahunnya di Indonesia. LES ditandai dengan adanya hiperaktivasi sel-sel imun yang menyebabkan proses penuaan sel imun (immunosenescence). Proses ini ditandai dengan menurunnya jumlah sel naif yang ditandai dengan marker CD45RA+ dan peningkatan jumlah sel memori dengan penanda CD45RO+. Sel-sel ini menyebabkan terjadinya produksi sitokin proinflamasi seperti IFN-γ secara terus menerus dan menyebabkan inflamasi kronik. Inflamasi kronik akan memicu sel B untuk menghasilkan antibodi anti ds-DNA. Tujuan dari penelitian ini adalah untuk mengetahui kaitan antara proses penuaan sel imun (immunosenescence) dengan derajat keparahan penyakit pada pasien Lupus Eritematosus Sistemik (LES). Subyek penelitian adalah pasien LES wanita usia 15-56 tahun yang memenuhi kriteria SLICC tahun 2012 dan berobat di Poli Reumatologi Penyakit Dalam Rumah Sakit Saiful Anwar (RSSA) Malang.. Sampel yang diambil adalah darah dari vena perifer. Variabel yang diukur adalah persentase sel T CD8+CD45RA+, sel T CD8+CD45RO+, kadar IFN-γ, dan kadar antibodi anti ds-DNA. Metode yang digunakan untuk mengukur kadar IFN-γ dan antibodi anti ds-DNA. Sedangkan, metode flowsitometri digunakan untuk mengukur jumlah sel T CD8+ CD45RA dan sel T CD8+ CD45RO+. Hasil penelitian dianalisis dengan uji normalitas Saphiro Wilk, uji homogenitas Levene test, dan uji korelasi Pearson. Persentase sel T CD8+CD45RA+ secara nyata lebih tinggi pada pasien dengan SLEDAI<5 (p=0.000) sedangkan persentase sel CD8CD45RO+ lebih tinggi pada kelompok dengan SLEDAI>5 (p=0.000). Kadar anti ds-DNA dan sitokin IFN-γ pada kelompok pasien dengan SLEDAI>5 lebih tinggi secara signifikan (p=0.000)(p=0.003). Adanya korelasi antara anti ds-DNA dan profil penuaan sel imun yaitu sel sel CD8CD45RA+ (p=0.020, r=-0.329) dan sel CD8CD45RO+ (p=0.010, r=0.270). Adanya korelasi antara sitokin IFN-γ dan sel viii T senesens yaitu dengan sel CD8CD45RA+ (p=0.02, r=-0.327) dan CD8CD45RO+ (p=0.045, r=0.284). Berdasarkan penelitian, kesimpulan dari penelitian adalah persentase sel T senesens berhubungan dengan meningkatnya anti ds-DNA, dan peningkatan kadar IFN-gama.
English Abstract
Systemic Lupus Erythematosus (LES) is a chronic autoimmune disease with increasing number of deaths each year in Indonesia. LES is characterized by the presence of hyperactivation of immune cells that leads to immune cell aging (immunosenescence). This process is characterized by a decrease in the number of naive cells with a CD45RA + marker and an increase in the number of memory cells with a CD45RO + marker. These process leads to the production of proinflammatory cytokine such as IFN-γ continuously and cause chronic inflammation. Chronic inflammation will trigger B cells to produce anti-ds-DNA antibodies. Aim of this study is to know relation between immunosenescence profile and severity of disease in SLE patients. Subjects of this study were SLE female patients with aged 15-56 years which were met the SLICC 2012 classification criteria. All of subjects were routinely came to Rheumatology Polyclinic at Saiful Anwar Hospital Malang. Blood of subjects were taken from the peripheral veins. The measured variables were IFN-γ, CD8 +CD45RA + T cells percentage, CD8 + CD45RO + T cells percentage, and anti-ds-DNA antibody levels. The method which were used to measure IFN-γ and anti-ds-DNA antibodies is ELISA. Method which were used for measure CD8 + CD45RA T cell percentage and CD8 + CD45RO + T cell percentage was flowcitometry . The results were analyzed by normality test of Klmogorov Smirnov, Levene test homogenity test, and Pearson correlation test. The percentage of CD8+CD45RA + cells significantly higher in patients with SLEDAI <5 (p = 0.000). Meanwhile, CD8+CD45RO+ T cells percentage was significantly higher in patients with SLEDAI >5 (p = 0.000). The anti ds-DNA and IFN-γ cytokines in the group of patients with SLEDAI> 5 were significantly higher (p = 0.000) (p = 0.003). x There is significant correlation between anti ds-DNA and CD8CD45RA+ (p=0.020, r=-0.329) and CD8+CD45RO+ (p=0.010, r=0.270). There is also positive correlation between IFN-gamma and CD8+CD45RO+ (p=0.045, r=0.284). Besides, there is negative correlation between IFN-gamma and CD8+CD45RA+ (p=0.02, r=-0.327) Based on research, the conclusion of the study is the percentage of senescence T cells associated with increased anti ds-DNA, and increased levels of gamma IFN.
