Abstract

Pneumoniaoadalah infeksioakut yang menyebabkan alveolioterisi pus atau cairan sehinggaomenimbulkan kesakitan saat bernapas, hipoksia, sepsis, sampai dengan kematian. Secara global, setiap 35 detik 1 anak meninggal karena pneumonia. DioIndonesia, pneumonia menyebabkan 14% kematian balita dan 80% batita. Diperlukan perhatian khusus untuk menangani pneumonia sesuai keparahan. NLCR mulai diminati sebagai parameter untuk memperkirakan keparahan pneumonia karena pemeriksaannya relatif murah dan mudah diakses. Penelitiantini bertujuantmengetahui hubungantNLCR dengan tingkattkeparahan pneumoniatanak di RSUD Dr. Saiful AnwartMalang. Penelitian crosstsectional ini melihat rekamtmedis pasien pneumonia usia 2-59 bulan di RSUD Dr. Saiful Anwar Malang dari Januari 2017 hingga Desember 2018. ConsecutivetSampling dilakukan sesuai dengan kriteria inklusitdanteksklusi sehingga mendapatkan 50 sampel yang diklasifikasikan menjadi 25 pasien pneumonia dan 25 pasien pneumonia berat berdasar Pedoman Tatalaksana Pneumonia Balita 2015. Nilai NLCR dihitung berdasar hasil pemeriksaan darahmrutin. Didapatkan rerata NLCR pada pneumonia berat 4,3160 dan pneumonia 2,0544 dengan p=0,105. KurvatROC menghasilkan AUC sebesar 0,634. Nilai cut-off NLCR terbaik adalah 1,785 dengan sensitivitas 64% dan spesifisitas 60%. Rasiotprevalens NLCR pada pneumoniayanak adalah 1,64. Sehingga disimpulkan NLCR pada pneumoniatberat lebihrtinggi dari pneumonia namun tidak berbeda signifikan. Pasientpneumoniatanak dengan nilai NLCR >1,785 lebih beresiko menderita pneumonia berat.

English Abstract

Pneumonia is an acute infection where alveoli is filled with pus or fluid, causing pain when breathing, hypoxia, sepsis, to death. Globally, every 35 seconds, 1 child dies of pneumonia. In Indonesia, pneumonia causes 14% of under-five deaths and 80% of toddlers. Special attention is needed to treat pneumonia according to severity. NLCR gain popularity as pneumonia severity parameter as the examination was relatively inexpensive and easily accessible. This study aims to determine the relationship between NLCR and pediatric pneumonia severity in RSUD Dr. Saiful Anwar Malang. This cross sectional study looked at medical records of pneumonia patients aged 2-59 months in RSUD Dr. Saiful Anwar Malang from January 2017 to December 2018. Consecutive sampling according to inclusion and exclusion criteria obtained 50 samples classified into 25 pneumonia patients and 25 severe pneumonia patients according to the Toddler Health Pneumonia Management Guidelines 2015. NLCR are calculated from routine blood tests. NLCR mean in severe pneumonia was 4.3160 and in pneumonia was 2.0544 (p = 0.105). The ROC curve yields an AUC of 0.634. The best NLCR cut-off value is 1.79 with a sensitivity of 64% and a specificity of 60%. Prevalence ratio of NLCR to pneumonia severity was 1.64. It was concluded that NLCR in severe pneumonia was higher than pneumonia but not significantly different. Pediatric pneumonia patients with NLCR>1.785 are more at risk of suffering from severe pneumonia.
