Abstract

Malaria pada kehamilan dapat menimbulkan komplikasi pada ibu dan juga pada janin berupa Berat Badan Lahir Rendah (BBLR). Mekanisme yang mendasari adalah sekuestrasi parasit yang dapat menimbulkan infiltrasi monosit dan pigmen malaria pada intervillous space. Penelitian ini dilakukan untuk mengetahui mekanisme Plasmodium vivax dalam patogenesis malaria plasenta. Penelitian ini adalah observasional analitik dengan pendekatan cross-sectional. Sampel jaringan plasenta didapat dari Maumere, Nusa Tenggara Timur. Sampel sejumlah 25 sampel terdiri dari 12 positif dan 13 negatif terinfeksi Plasmodium vivax yang dikonfimasi melalui pemeriksaan Polymerase Chain Reaction (PCR). Sampel jaringan plasenta dilakukan pengecatan Hematoksilin-Eosin dan diamati dengan perbesaran 1000x pada 100 lapang pandang. Dilakukan penghitungan densitas parasit (sekuestrasi eritrosit terinfeksi), infiltrasi monosit, dan pigmen parasit. Pengamatan mikroskopis menemukan terdapat infiltrasi monosit dan akumulasi pigmen parasit malaria pada kelompok positif dan negatif dan tidak ada perbedaan yang signifikan antara kedua kelompok (p=0,583; p=0,723). Uji korelasi menunjukkan bahwa tidak ada hubungan yang signifikan antara infiltrasi monosit dan BBLR (p=0,626; p=862) serta antara pigmen parasit dan BBLR pada kedua kelompok (p=0,324; p=0,181). Selain itu juga tidak ada korelasi yang signifikan antara densitas parasit dan BBLR pada kedua kelompok (p=0,404; p=0,064) meskipun terdapat perbedaan signifikan pada densitas parasit pada plasenta antar kelompok (p=0,000). Dari penelitian ini dapat disimpulkan bahwa infeksi Plasmodium vivax menyebabkan plasenta akut, subakut, dan kronis pada ibu hamil terinfeksi subklinis di RSUD TC Hillers, Maumere, Nusa Tenggara Timur yang menyebabkan bayi BBLR.

English Abstract

Malaria in pregnancy can cause complications to mother and fetus such as Low Birth Weight (LBW). The mechanism is based on the presence of parasite sequestration which can cause monocyte infiltration and malaria pigment in the intervillous space. This study was conducted to determine the mechanism of Plasmodium vivax in the pathogenesis of placental malaria and its relationship wih fetal low body weight. This study was observational analytic with a crosssectional approach. Placental tissue samples obtained from pregnant women who has low birth weight baby during delivery in Maumere, Nusa Tenggara Timur. Sample used in this study were 25 samples consisted of 12 positive and 13 negative samples. Plasmodium vivax-infected samples were confirmed by Polymerase Chain Reaction (PCR). Placental tissue samples made with Hematoxylin-Eosin staining and observed under 1000x magnification at 100 field using light microscope. Sequestration of Plasmodium-infected erythrocytes (parasite density), monocyte infiltration, malaria pigments accumulation were calculated. Microscopic observation revealed monocyte and malaria pigments accumulation both in positive and negative groups and no significant difference between groups (p=0.583; p=0.723). Correlation test showed that there was no significant relationship between monocyte infiltration and LBW in the positive group and negative group (p=0.626; p=862) as well as between parasitic pigments and LBW in both groups (p=0.324; p=0.181). Moreover there was also no significant correlation between parasite density and LBW in the positive and negative group (p=0.404 ; p=0,064) although there was a siginficant diferrent of placenta parasite density between groups (p=0,000). From this study it can be concluded that Plasmodium vivax infection cause acute, sub acute and chronic malaria placenta in subclinically infected pregnant women in Maumere, Nusa Tenggara Timur that might cause LBW baby.
