Abstract
Latar Belakang: Akses intravena perifer merupakan suatu prosedur tindakan yang paling sering dilakukan pada instalasi gawat darurat. Pemasangan kanul vena perifer dengan metode konvensional sering mengalami kegagalan pada pasien dengan vena tidak teraba, meskipun dilakukan oleh tenaga medis terlatih. Perlu adanya inovasi kanulasi vena perifer dengan tingkat keberhasilan yang lebih besar, salah satunya menggunakan ultrasonografi. Tujuan: membandingkan efektivitas (jumlah usaha, waktu, biaya) serta komplikasi yang muncul dalam mendapatkan akses intravena perifer antara metode konvensional dengan ultrasonografi pada pasien dengan akses vena tidak teraba. Metode: penelitian true experimental klinis dengan pendekatan klinis post test only control group design single blinded pada 40 sampel penelitian di IGD Rumah Sakit Saiful Anwar untuk membandingkan efektivitas (jumlah usaha, waktu, biaya) serta komplikasi yang muncul dengan metode kanulasi intravena perifer. Hasil: Kanulasi menggunakan ultrasonografi lebih efektif dibandingkan dengan metode konvensional dalam kanulasi vena perifer ( p < 0.05) namun dengan biaya yang lebih mahal (p < 0.05) Kesimpulan: Kanulasi vena perifer dengan panduan ultrasonografi lebih efektif, sedikit menimbulkan komplikasi namun lebih mahal dibandingkan metode konvensional
English Abstract
Background: Peripheral intravenous access is the most frequent procedure of action in department of emergency medicine. The insertion of peripheral venous by conventional method often fails in patients with impalpable veins, although performed by trained medical personnel. There is a need to improve peripheral venous cannulation method with greater success rate specifically in patients with impalpable veins, one of them using ultrasonography as a guide. Objective: The aim of this study is to compare the effectiveness (number of attempts, time, and cost) as well as the emerging complications in obtaining peripheral intravenous access between conventional and ultrasonography guided method in patients with impalpable venous access. Method: A true clinical experimental study with a single blinded posttest only control group design approach on 40 samples of the study at the Department of Emergency Medicine, Saiful Anwar General Hospital to compare the effectiveness (number of attempts, time, costs) and complications arising from peripheral intravenous canulation using conventional and ultrasonography guided method. Results: The peripheral venous cannulation using ultrasound is more effective than conventional method both in terms of number of attempts, time and emerging complications (p <0.05) but with higher cost (p <0.05) in patients with impalpable veins. Conclusion: Peripheral venous cannulation with ultrasound guidance is more effective in terms of number of attempt and time to cannulate, less side effect but more expensive than conventional method for peripheral venous cannulation in patients with impalpable veins.
