Abstract

Karsinoma nasofaring (KNF) merupakan penyumbang kasus kanker kepala dan leher terbanyak, dan berada pada urutan ke-4 terbanyak di Indonesia. Karsinoma nasofaring memiliki probabilitas metastasis yang lebih tinggi dibandingkan karsinoma sel skuamosa kepala leher lainnya. Beberapa molekul adhesi termasuk E-cadherin diakui berpotensi sebagai biomarker kejadian metastasis tumor. Studi terbaru menemukan misregulasi miRNA-9 pada beberapa jenis sel kanker yang mengarah pada terjadinya metastasis. Penelitian ini bertujuan untuk mengetahui hubungan E-cadherin jaringan dan mikro RNA 9 darah dengan ukuran nodul metastasis di leher. Penelitian ini adalah penelitian observasional analitik dengan pendekatan cross sectional. Data yang terdiri dari 26 subyek penelitian pasien KNF WHO Tipe 3 dengan LMP-1 Positif yang belum menjalani terapi dari tahun 2017-2018 di poliklinik THT RSUD dr. Saiful Anwar Malang diperoleh secara consecutive sampling. Spesimen jaringan diperoleh melalui biopsi nasofaring. Sampel serum diperoleh dari darah vena mediana cubiti. Karakteristik sampel dan ukuran nodul leher diperoleh dari data logbook terapi pasien kanker poli THT RSUD dr. Saiful Anwar. Deteksi ekspresi LMP-1 dan E-cadherin dari spesimen jaringan menggunakan metode imunohistokimia. Status terdeteksinya ekspresi mir-9 dideteksi dengan menggunakan instrumen RT-qPCR Thermal Cylcer CFX96 (BioRad). Melalui uji Chi Square (p=0.804) didapatkan data bahwa tidak ada perbedaan yang bermakna proporsi antara status terdeteksinya ekspresi miRNA-9 serum dengan ukuran nodul metastasis (nilai N). Uji Kruskal Wallis (p=0.003) menunjukkan perbedaan yang signifikan antara ekspresi E-cadherin jaringan pada berbagai kelompok ukuran nodul metastasis di leher (nilai N). Uji Mann Whitney menunjukkan perbedaan yang signifikan antara kelompok N1 dengan N3 dan kelompok N2 dengan N3. Namun tidak ada perbedaan signifikan antara kelompok N1 dengan N2. Uji korelasi Spearman menunjukkan koefisien korelasi sebesar -0.488, berarti terdapat hubungan negatif yang cukup kuat antara ekspresi E-cadherin dengan volume nodul. Dapat disimpulkan bahwa semakin rendah ekspresi E-cadherin jaringan, semakin besar volume nodul di leher

English Abstract

Nasopharyngeal carcinoma (NPC) is the largest contributor to head and neck cancer, and ranks 4th most commonly diagnosed cancer in Indonesia. Nasopharyngeal carcinoma has a higher metastatic probability than other neck and head squamous cell carcinomas. Several adhesion molecules including Ecadherin are recognized as potential biomarkers of tumor metastatic events. The latest study found misregulation of miRNA-9 in several types of cancer cells that lead to the occurrence of metastasis. This study aimed to determine the relationship of E-cadherin tissue and blood micro RNA 9 with the size of metastatic nodules in the neck. This study was an observational analytic study with a cross sectional approach. The data consisted of 26 research subjects in WHO Type 3 NPC patients with Positive LMP-1 who had not undergone therapy from 2017-2018 in the ENT polyclinic of the RSUD dr. Saiful Anwar Malang was obtained by consecutive sampling. Tissue specimens were obtained through nasopharyngeal biopsy. Serum samples were obtained from the mediana cubiti vein blood. Sample characteristics and size of neck nodules were obtained from logbook data on therapy of ENT polyclinic cancer patients at RSUD dr. Saiful Anwar. Detection of LMP-1 and E-cadherin expression from tissue specimens was carried out using immunohistochemical methods. The detection of the expression mir-9 status was detected using the Thermal Cylcer CFX96 (BioRad) qPCR instrument. The Chi Square test (p = 0.804) shows that there is no significant difference in the proportion between the detected status of serum miRNA-9 expression and the size of metastatic nodules (N value). The Kruskal Wallis test (p = 0.003) shows significant difference between tissue E-cadherin expression in various categories of metastatic nodule size in the neck (N value) (Kruskal Wallis, p = 0.003). The Mann Whitney test shows a significant difference between groups N1 and N3 and groups N2 with N3. However, there is no significant difference between the N1 and N2 groups. The Spearman correlation test shows a correlation coefficient of -0.488, meaning that there is a moderately strong negative relationship between the expression of E-cadherin and the volume of nodules. It can be concluded that the lower the expression of tissue Ecadherin, the greater the volume of nodules in the neck
