Abstract
Skizofrenia merupakan salah satu gangguan mental serius yang mengubah persepsi, emosi, kognisi, pemikiran, dan tingkah laku penderitanya. Salah satu gejalanya yaitu gejala negatif yang ditandai dengan hilang/berkurangnya sifat yang seharusnya terdapat pada individu normal. Namun masih banyak penderita skizofrenia yang tidak langsung mendapatkan terapi karena pengobatan dan akses ke pelayanan kesehatan jiwa yang belum memadai. Kejadian ini disebut ‘duration of untreated psychosis’ (DUP) atau durasi psikosis tanpa terapi. Tujuan dari penelitian ini adalah untuk mengetahui apakah terdapat perbedaan keparahan gejala negatif menggunakan modifikasi Scale for Assessment of Negative Symptoms (SANS) berdasarkan durasi psikosis tanpa terapi pada pasien skizofrenia. Penelitian dilakukan di RSUD Dr. Saiful Anwar Malang dengan jumlah sampel 48 orang. Metode yang digunakan yaitu menggunakan penilaian modifikasi SANS sebagai instrumen penelitian. Dari anamnesis pasien kemudian dilakukan klasifikasi pada DUP sehingga terbagi menjadi DUP < 1 tahun dan DUP > 1 tahun. Analisis data yang digunakan yaitu Mann-Whitney. Berdasarkan hasil uji komparatif numerik menunjukkan nilai p 0,019 (p < 0.05), sehingga dapat disimpulkan bahwa terdapat perbedaan signifikan pada keparahan gejala negatif menggunakan modifikasi SANS berdasarkan durasi psikosis tanpa terapi. Apabila durasi psikosis tanpa terapi semakin lama, maka gejala negatif juga akan semakin buruk yang dinilai dari skor modifikasi SANS yang semakin meningkat.
English Abstract
Schizophrenia is one of the serious mental disorders that changes perceptions, emotions, cognitions, thoughts, and behaviors of sufferers. One symptom is a negative symptom that is characterized by loss / reduction in traits that should be present in normal individuals. But there are still a lot of schizophrenics who don't get immediate therapy because of treatment and access to inadequate mental health services. This event is called "duration of untreated psychosis" (DUP). The purpose of this study was to determine whether there were differences in the severity of negative symptoms using a modified Scale for Assessment of Negative Symptoms (SANS) based on the duration of untreated psychosis in schizophrenic patients. The study was conducted at the RSUD Dr. Saiful Anwar Malang with a sample of 48 people. The method used is using the modified SANS assessment as a research instrument. From the patient's history, the DUP was classified so that it was divided into DUP < 1 year and DUP > 1 year. Analysis of the data used is Mann-Whitney. Based on the results of the numerical comparative test showed a p-value of 0.019 (p < 0.05), so it can be concluded that there were significant differences in the severity of negative symptoms using a modified SANS based on the duration of untreated psychosis. If the duration of untreated psychosis is prolonged, the negative symptoms will also get worse, judging by the increasing SANS modification score.
