Abstract
Diabetes Mellitus tipe 1 menjadi masalah kesehatan di banyak negara, dengan keseluruhan peningkatan per tahun diperkirakan sebesar 3%. DM tipe 1 merupakan konsekuensi dari proses destruksi autoimun sel β. Ditemukan hubungan antara defisiensi vitamin D dengan DM tipe 1. Proses destruksi terjadi akibat ketidakseimbangan sitokin pro inflamasi dan anti inflamasi. Salah satu sitokin pro inflamasi adalah IL-2. Pemeriksaan C-peptida untuk melihat fungsi dari sel beta akibat destruksi sel beta pankreas. Pemberian suplementasi vitamin D3 masih menimbulkan kontroversi dan memberikan hasil yang bervariasi. Penelitian randomized clinical trial ini dilakukan untuk mengetahui kadar 25(OH)D3, IL-2, dan C-peptida pada penderita DM tipe 1 yang mendapatkan suplementasi vitamin D3. Subyek penelitian adalah 26 anak dengan DM tipe 1, yang terbagi menjadi kelompok K1 (mendapatkan suplementasi vitamin D3) dan kelompok K2 (mendapatkan plasebo). Hasil penelitian menunjukkan kadar 25(OH)D3 yang lebih tinggi pada kelompok K1 dan secara statistik didapatkan perbedaan yang bermakna (p=0,000). Didapatkan kadar IL-2 yang lebih tinggi dan C-peptida yang lebih rendah pada kelompok K1 dan secara statistik tidak didapatkan perbedaan kadar yang bermakna (p=0,762 dan p=0,264). Hubungan yang tidak signifikan dan korelasi yang negatif didapatkan antara kadar 25(OH)D3 dan IL-2 (p=0.707; r= - 0.115), kadar 25(OH)D3 dan C-peptida (p=0,593; r= -0.164), serta kadar IL-2 dan C-peptida (p=0.132; r= -0.441) pada anak DM tipe 1 yang mendapatkan suplementasi vitamin D3. Dari penelitian ini dapat disimpulkan bahwa pemberian suplementasi vitamin D3 pada anak dengan DM tipe 1 dapat meningkatkan kadar 25(OH)D3 secara signifikan. Peningkatan kadar 25(OH)D3 tidak secara signifikan menurunkan kadar IL-2 dan meningkatkan kadar C-peptida. Namun, didapatkan tingkat absolut penurunan C-peptida terjadi lebih lambat pada kelompok yang mendapatkan suplementasi dibandingkan yang mendapatkan plasebo.
English Abstract
Type 1 Diabetes Mellitus has become a health problem in many countries. Type 1 DM is the consequence of autoimmune destruction process of β cells. There was relationship between vitamin D deficiency with Type 1 DM. The destruction process was caused by an imbalance of pro-inflammatory and anti-inflammatory cytokines. One of the pro-inflammatory cytokines is IL-2. Cpeptide examination to see the function of beta cells due to destruction of pancreatic beta cell. Administration of vitamin D3 supplementation still cause controversy and give varying results. This randomized clinical trial was conducted to determine the levels of 25(OH)D3, IL-2, and C-peptide in people with type 1 diabetes who received vitamin D3 supplementation. The subjects were 26 children with type 1 DM, divided into K1 group (received vitamin D3 supplementation) and K2 group (received placebo). The results showed higher levels of 25(OH)D3 in the K1 group and statistically found a significant difference (p= 0.000). Higher levels of IL-2 and lower C-peptide were obtained in the K1 group and no statistically significant differences were found (p= 0.762 and p= 0.264). The insignificant relationship and the negative correlation were found between 25(OH)D3 and IL-2 (p= 0.707; r= - 0.115), 25(OH)D3 and C-peptide (p= 0,593; r= -0.164 ), also levels of IL-2 and C-peptide (p= 0.132; r= -0.441) in children with type 1 diabetes who received vitamin D3 supplementation. From this study can be concluded that administration vitamin D3 supplementation in patients with type 1 DM can increase levels 25(OH)D3 significantly. This increase has not significantly lowered levels of IL-2 and increased levels of C-peptide. However, there was an absolute decrease in the rate of slower C-peptide in the supplemented group than in the placebo group.
