Abstract

Latar Belakang: Tuberkulosis paru dan tuberkulosis paru resisten obat merupakan masalah kesehatan dunia. Dari semua orang yang terinfeksi tuberkulosis hanya sebagian kecil berkembang menjadi sakit tuberkulosis. Masih belum jelas penyebab mengapa seseorang yang terinfeksi tuberkulosis bisa menjadi sakit sementara yang lainnya bisa bertahan dari infeksi tersebut. Faktor genetik dipercaya mempengaruhi kerentanan seseorang terhadap infeksi tuberkulosis. Interleukin 17 merupakan sitokin yang terlibat pada fase akut dan kronis. Sitokin ini berperan penting dalam pathogenesis infeksi tuberkulosis. Tujuan: Mengetahui hubungan polimorfisme interleukin 17F rs 763780 dengan kerentanan terhadap tuberkulosis paru dan tuberkulosis paru resisten obat. Metode: Penelitian case control dari September 2017 sampai Januari 2018 melibatkan 80 orang yang terdiri dari 27 penderita tuberkulosis paru, 27 penderita tuberkulosis paru resisten obat dan 26 subjek sehat di Malang Indonesia. Analisis genotip menggunakan metode RFLP (Restriction Fragment Length Polymorphism). Hasil: Individu dengan alel C meningkatkan kerentanan terhadap tuberkulosis paru (OR 15.1; 95% CI = 3.55 – 64.21) dan tuberkulosis paru resisten obat (OR 6.61; 95% CI = 1.96 – 22.27). Genotip CC (homozigot) lebih beresiko lebih besar 3 kali lipat dibanding genotip CT (heterozigot) pada kelompok tuberkulosis paru sensitif obat, sedangkan pada kelompok tuberkulosis resisten obat resiko genotip CC meningkat 6 kali lipat dibandingkan genotip CT. Kesimpulan: Polimorfisme IL-17F rs 763780 alel C meningkatkan kerentanan terhadap tuberkulosis paru dan tuberkulosis paru resisten obat.

English Abstract

Background: Pulmonary tuberculosis and drug resistant pulmonary tuberculosis are a global health problem. Of all people infected with tuberculosis only a small proportion develops into tuberculosis. It remains unclear why a person who is infected with tuberculosis can become ill while others can survive the infection. Genetic factors are believed to affect a person's susceptibility to tuberculosis infection. Interleukin 17 is a cytokine involved in the acute and chronic inflammatory phase. This cytokine plays an important role in the pathogenesis of tuberculosis infection. Objective: To determine the relationship of interleukin polymorphism of 17F rs 763780 with susceptibility to pulmonary tuberculosis and drug resistant pulmonary tuberculosis. Method: The case control study from September 2017 to January 2018 involved 80 people consisting of 27 pulmonary tuberculosis patients, 27 patients with pulmonary resistant tuberculosis and 26 healthy subjects in Malang Indonesia. Genotype analysis using RFLP (Restriction Fragment Length Polymorphism) method. Results: Individuals with C allele increased susceptibility to pulmonary tuberculosis (OR 15.1, 95% CI = 3.55 - 64.21) and drug resistant pulmonary tuberculosis (OR 6.61; 95% CI = 1.96 - 22.27). The CC (homozygous) genotype was at greater risk 3 times greater than the CT (heterozygous) genotype in the drug-sensitive pulmonary tuberculosis group, whereas in the drug-resistant tuberculosis group the CC genotype risk increased 6-fold compared with the CT genotype. Conclusions: IL-17F rs 763780 allele C polymorphisms increase susceptibility to pulmonary tuberculosis and drug resistant pulmonary tuberculosis.
